Hi Doctor, I'm 27 And I Don't Have Any Chronic

default

Question: hi doctor,

i'm 27 and i don't have any chronic diseases but ever since March I've been having inconsistent erection issues. I get these inconsistencies in all aspects from lack of morning wood to masturbation to physically being with somebody.

Many of my doctors don't take my issue seriously because they say it is very unlikely to have erection issues at 27 but I just feel that my penis feels different even while flaccid. Maybe i am thinking about it too much but i am afraid i do have some sort of physical erection issue. i was even reading an article that unike what doctors thought before that most ED in young men is not psycholoigcal.

my doctor took a testosterone test and that came back normal. i am trying to get a penile doppler test ordered since that would confirm or deny everything once and for all but my doctors are not ordering it.

what do u think the most likely cause is?
is a venous leak a strong possibility?

Detailed Answer:
Hello!

Welcome to Ask a Doctor service!

Regarding your concern, I would explain that your symptoms don't seem to be related to any leaking vein as it would be really exceptional (they are usually born defects and you would have detected it since childhood).

In my opinion, your symptoms could be related to a hormonal imbalance.

Even though testosterone levels are normal, I would recommend checking also prolactin plasma levels, thyroid hormone levels and other androgens levels.

A spermogram ( analysis of your sperm) would give some further information too.

If all the above tests result normal, you should consider consulting with a psychiatrist or psychotherapist. There are many drugs that can help improve your situation, when there is a psychological problem and all the other tests result normal.

Hope you will find this answer helpful!

Kind regards,

Dr.Ilir Sharka, Cardiologist

i think i was so traumatized by finding out i had phimosis i literally thought about it 24/7 during the 1 week i found i had phimosis. it may have caused emotional distress to my sex life in general.

i didn't do a full hormone test but i did do a semen analysis and everyhting came back normal:

113 million sperm count in total
9% morphology
44% motility with 2.5 forward progression

so is venous leak 1 in a million or something? the only reason i even thought about venous leak is because i read that it is possible to have venous leak ED as a young adult vs all the other causes would be if you are over 50.

i am a hypochondriac so myabe i do seem to over think things but isn't it also possible that a hypochondriac is right with at least one of their synptoms? in prior years i freaked out about having hiv, herpes, appendicitis, infertility, etc. but after getting tested for these they all came back negative.

this is th only time where i am freaked out for over 8 months...it may be due to the fact that i want a doppler test.

plus it is super concernning that there is no cure for ED. at least if i knew that there was conservative management for ED i would feel better but it seems that all the treatment options for ED especially for young adults as myself are realy crappy (no offense). If i have physical ED i dont want my only options to be to take a fucking pill every time i want to have sex or masturbate or to get an implant. that;s so embarassing and honestly so bad for my mental health. yes, ed may not be a life or death disease but at 27 years old if i find out i have physical ed with no possible cure then i would be left completely suicidal. i swear id rather have cancer at 27 than ed.

its just not very reassuring that there is no cure for this and even if i have poor arteries and veins that i have to get surgery to correct it. its been stressing me out since march.

My opinion as follows:

Detailed Answer:
Hello again!

I understand your concern and would explain that it seems really less possible that you have a leaking vein.

In my opinion, considering your past history and your fears, it seems to be just a psychological issue.

The fact that all your tests came back normal, is very reassuring that nothing serious is going on. It is true that it can be really distressing to live with this issue, but you should know that there is still hope, as there are many treatment options ( mainly antidepressants, which are used to treat erectile dysfunction).

Phymosis is a common disorder in young patients and it can trigger all this clinical situation. If treated properly with surgery it can resolve the problem.

Besides, the leaking vein can be diagnosed with an ultrasound of the testicles. i recommend performing it, just to calm your anxiety.

I recommend practicing recreational sports and aerobics, which can help manage anxiety and improve your situation.

In my opinion psychological stress is playing an important role in all this clinical situation and you should try to manage it as much as possible.

In this regard, consulting with a psychiatrist can help improve your situation.

Hope to have been helpful!

I remain at your disposal for any further questions!

Wishing all the best,

i know you say that there are many treatment options but i am bit interested in those options. the only thing i am interested in is going back to having normal erections again.

and the only tests i have taken were a blood test and a semen analysis. i took a stendra and got a weak erection while masturbating...

also, is the doppler ultrasound an ultrasound for the penis or the testicles?

and i dont want to get surgery for my phimosis. i dont want to be circumsized. i just want to have a normal foreskin. how hard could that be?

is there any new research on ED and permantely curing it? it looks like the only option men with venous leak have is to get an implant which is so embarassing and not idea at all. i've read about young men who commmit suicide because of it. its a very serious issue.

plus, if this was really psychological why would my morning erections be inconsistent? i used to get morning wood every day. i'm only 27...why would i be having less morning erections if it wasnt a physical problem.

Dear XXXXXXX

The ultrasound and doppler ultrasound are the same test. I don't think that you have a venous leak as it would have been detected by the doppler ultrasound.

Regarding the morning erection, you should know that psychological disorders can cause this too.

For this reason, I would recommend consulting with a psychiatrist in order to help you with drugs (antidepressants, which improve erectile dysfunction). These drugs are taken for a short period of time (some months regularly) and then stopped after you gain your normal erectile function. These drugs do not need to be taken forever or in every sexual intercourse.

Regarding the phymosis, there is no need for circumcision. You can try plastic surgery to improve the urine output.

You should discuss with your doctor on the above treatment options.

Well I never got a Doppler test done do a venous leak or any other artery problem isn’t ruled out. Are there any other symptoms of a venous leak that I can find out without taking the Doppler test? Like pain in the penis or shrinkage if penis or something like that?

Also, is Stendra supposed to be a strong drug for erections? How come I took it last month and got a weak erection? I had stronger erections naturally after that.

Once I know 10000% that there is no physical or hormonal problem I will be reassured that this is psychological and hopefully that will help with my erection function.

As far as the phimosis, I don’t have a urine output problem. I can retract my foreskin all the way when flaccid but only past my glans when erect. So basically I have partial phimosis and I am not interested in surgery.

Hello again XXXXXXX

Yes, a Doppler ultrasound would help to exclude the diagnosis of venous leaking.

Stendra is a strong drug for erections, but sometimes, it may not work.

Regarding the symptoms of venous leaking, other symptoms would be shrinkage of the penis without pain, loss of penile length and girth, and a soft glans of the penis during erection that is not fully engorged.

Hope you will find this information helpful!

well my penis for the most part is the same size. sometimes it flucuates when it is cold or after i excersize at the gym but generally it doesn't seem smaller.

is anxiety seriously that strong where it would effect erections 24/7? it isnt just during sex or physical contact.i used to lay in my bed and get an erection the entire night and right before i fell asleep without even using my hands. i could just feel myself movng my erectile muscles into becoming bigger and giving me an erection if that makes sense? now i sleep with a flaccid penis and it just feels very different.

is it possible at all that a young healthy male would have PHYSICAL erection issues without any other condition such as peyrinies disease, diabetes, epilepsy, spinal cord surgery or anything else? i am just wondering what could be causing this other than anxeity.

i am not really too sure if i want to take the doppler test for two reasons:: 1. i dont want to be injected with whatever injection they use since im scared it will cause harm or permament side effects to m erections and penis and 2. if they do find that i have a blood flow issue then i will be EXTREMELY depressed and maybe even suicidal since i dont want to live my entire young life not being able to get a hard erection naturally. at least if there was a chance of curing or reversing physical ED i would feel hopeful but i feel depressed thinking about this because the only options doctors have is to get a penile implant and that would be extremly embarassing. it just seems like a really stupid option to get one. oral medications wont dothe trick either since i want NATURAL erections, not ones I have to take medication to get. I keep trying to think of possible causes and i cant think of anything. i have no other diseases that would effect my penis. its just so frustrating

Hello again!

I understand how you feel, but you should know that persistent anxiety can lead to depression and depression can mimic all this clinical situation (low libido, erectile dysfunction, etc.).

There are many young patients who suffer ED, even in the absence of any other diseases. This is because people are really stressed today and life is really difficult.

Regarding the Doppler ultrasound, they do not inject any substance. It is just a normal ultrasound with a specific technique. There is no injection or intervention during the test.

Regarding drugs, I don't think you will take any drugs like Stendra as often as you will have sex. But, an antidepressant, can help improve the way you feel and thus improve ED and your libido or fears.

Hello again!

I can not say that it is just anxiety, without a Doppler ultrasound.

Besides, depression is not always understood from the patients. It can cause dysfunction in some domains (sexual function, libido, etc.).

From the other hand, antidepressants are used to treat ED even in the absence of depression.

But, I would also recommend checking thyroid hormone levels in order to exclude a possible thyroid gland dysfunction.

Regarding disorders of the spinal cord, they would have other symptoms like gait disturbances, urinary retention, numbness in the legs or motor deficits in the legs. So, I would exclude such possibility.

The same is for diabetes. It would show up in the lab tests and it would also be associate to other symptoms like: increased thirst and urination during night, weight loss, etc..

Hope to have been helpful!

so are you 100% sure that this is due to anxiety? i do have persistent thoughts about this but i dont think i am to te point of having depression.

is the only way to have physical erection issues when you're young to also have one of the linked diseases like diabetes or peterines diseases or spinal chord injury? if i don't have any other one of these diseases is it almost gauranteed that nothing is physically wrong? i just want to make sure that this isnt just happening for no reason.

So basically you’re saying from what I’m stating that there is a chance that this sounds like a physical problem and therefore I should go in for an ultrasound? Just want to clarify.

Have there been a lot of cases of young healthy men with organic ED?

Also, do u know if research for erectile dysfunction is currently progressing to find better treatments? The treatment options that are currently available do not sound good to me.

And antidepressant medicines have a side effect that can cause ED don’t they?

My answer as follows:

Detailed Answer:
Dear friend,

The number of young men with ED has been increasing in a high speed in the last years. So, you are not alone or considered a rare case.

The doppler ultrasound is necessary to examine the integrity of the vascular mechanism underlying erection. It can help determine if there is any physical abnormality (including a leaking vein).

The next thing would be to check your hormones:

- thyroid hormone levels
- Prolactine plasma levels for pituitary gland dysfunction
- LH plasma levels
- cortisol and aldosterone plasma levels for adrenal gland dysfunction
- androgene plasma levels.

Many antdiepressants can cause erectile dysfunction. But some of them, can help improve the situation, if the mechanism underlying erectile dysfunction is depression or anxiety.

The number of young patients with ED is increasing because of many factors: poor lifestyle (drug abuse, smoking, alcohol intake, obesity, etc.), increased stress in daily life.

Your symptoms do not sound like organic. Anyway, a Doppler ultrasound would certainly help clarify this issue.

I would recommend trying Viagra or Cialis for a better result.

but are those increasing numbers of ED in young men psychological or physical?

in your opinion, do my symptoms sound like i have a physical problem?

i already got all of my hormones checked and they all came back normal. the only thing left is the ultrasound.

is there any new research on new ED treatments rather than what we have now? I would feel better if there was better treatments or better cures.

so it sounds like he increasing number of ED are all reversible causes correct? Like changing your lifestyle or lowering stress.

I'll see if my doctor will order the test for me.

Just thinking about taking viagra or cialsis or any erection medication makes me more stressed and annoyed because i dont want to depend on those to get or keep an erection. I want my body to work the XXXXXXX way as other 27 year old bodys do and I don't want to be one of the few who needs medication to get an erection.

so it sounds like he increasing number of ED are all reversible causes correct? Like changing your lifestyle or lowering stress.

Hello again!

Coming to this point, I would recommend performing the Doppler ultrasound, which will definitely rule out a possible physical problem, which could be related to the stretching exercises.

Besides, there are many treatment options besides drugs like penile injections, etc., which can help improve your situation.

If a leaking vein is diagnosed, surgery would help to definitely solve the problem.

You are too young to think that your sexual life is over, because there are so many treatment options and you should try to be more optimistic. Science is really developed in this field and I am sure that you will find a solution soon, after performing the right tests and consulting with a specialist of this field.

You should discuss with your doctor on the above issues.

Hope to have been helpful!

I don't know why ever doctor is saying those drugs will cause high self esteem. Personally, if I take these drugs and see that the ONLY time I can get an erection is when I am on medication then that would not cause higher self esteem for me. To me, I would feel even more depressed knowing that I am 27 and need to take ED pills because I can't get them naturally.

I'm basically just asking how I can reverse this to make my erections normal again, not just during sex. I think when I discuss this with doctors they think the only time I am depressed is when I am trying to perform during sexual inercourse but I want to have erections during masturbation as well and have random erections like I used to like just sitting down and having a random erection eith no stimualtion etc.

I am just so annoyed that this is such a complex issue to diagnose and to cure or treat. Sex is very important for mental health and everything so I don't want to be 27 and having a physical erection problem it's super annoying and the treatment options for this are annoying as well. I honestly can;t wait any longer to have proper erections again. What if I am stuck like this forever? Or the conditions gets worse by the time I reach my 30's? It just doesn't make any sense why this would be occurring. I would like to think this is all due to anxiety but so many men in the world have a stressful life and are anxious and they don't have ED issues for 9 consecutive months.

I honestly think this is happening because of the foreskin stretching excersizs I did back in March to cure my phimosis. It can't just be a coinicidence that the erection changes happened 1 week after I tried doing the stretching excersizes.

do you know how it can be related to the stretching excersizes?

the only solution I want is just to have a normal functioning penis again. I understand it is developed but it isn't developed enough where every time I want a hard erection I need to do a painful injection or get an implant in my penis. I don't think anybody my age would be okay with this options....

Hello again!

Yes, I agree with your that injections and implants would not be the optimal choice in your case, as they are usually used in older persons, suffering from diabetes, etc..

I don't think that stretching exercises have lead to this condition. Anyway, a Doppler ultrasound would help exclude any damage to your penis or vascular mechanism in it.

Anyway, there are many types of physical exercises (including Kegel and Pilates exercises), which can help improve ED.

Regular physical activity and recreational sports can have a great impact on ED too. For this reason, I would strongly encourage you to follow them.

Hope you will find this information helpful!

Hi Doctor, I'm 27 And I Don't Have Any Chronic

hi doctor, i'm 27 and i don't have any chronic diseases but ever since March I've been having inconsistent erection issues.
