Hello and welcome to ask a doctor service. I have gone through your query and I have noted your concern.

I request you to kindly upload a few Images of the affected area in "Reports section" so that i am able to have a look and guide you accordingly.
White patches could be due to several causes like vitiligo, pityriasis XXXXXXX pityriasis versicolor etc. Fungal infection though is easy to diagnose and usually presents as itchy red patches with raised scaly borders which extends peripherally. Still, treatment of fungal infection (with oral and topical antifungal) depends on how extensive is the infection

Regarding onychomycosis and vitiligo

Detailed Answer:
Thank you. I have viewed the images.
You seem to have fungal infection of nail (onychomycosis) for which i would first suggest a confirmatory test with KOH nail clipping. Once confirmed, i advuce you to take an oral antifungal e.g Terbinafine 250mg once a day for 3-6 months. You may also put a topical antifungal e.g Amorolfine or Ciclopirox nail lacquer on affected nails.
The white patches seem to be those of vitiligo. If this is the only affected area, you may apply tacrolimus 0.1% ointment, twice daily. You may also use a topical psoralen followed by sunlight exposure under doctor's supervision.
