-Where is the pain? Is it on the upper back/lower back or any other parts?

-How bad is the pain (on a scale of 1-10, 10 being the worst)? Any tingling/numbness over the arms/fingers, any problem bending the neck?

-How is the pain? Is it constant or coming and going?

-Is the pain remaining constant in one area or spreading to somewhere else as well?

-Any recent history of trauma/injury to the back?

-Have you taken any medications yet?
