Question: Yesterday I got my biopsy report, basal cell carcinoma. I consulted dermatologist due to slight swelling behind my right calf which also bled at times, it's been there for more than 2 months. The biopsy report is as follows

Nature of specimen: Excised swelling

Gross Specimen:Skin with underlying fat measuring 1.3*1*0.8cms. This shows a ulcerated well circumscribed macule measuring 0.7cm diameter well away from all margins. Cut surface blackish. Entire specimen processed.

Microscopy:Sections show ulcerated epidermis with an underlying circumscribed neoplasm arranged in interconnecting trabeculae, cribriform pattern and clusters with intervening myxoid stroma . These cells are uniform polygonal showing increased N/C ratio and granular chromatin and scanty cytoplasm. Peripheral palisading is seen. Melanin incontinence and stromal retraction is seen. Lesion extends to deep margin.

Should further treatment be taken, or is the excision enough? Should i be worried , is it dangerous? Kindly reply. I will be visiting my dermatologist on Tuesday.

Basal cell carcinoma usually can be cured by surgery. But in this specimen, cancer cells extend to the deep margin. Hence the excision is not enough. I would recommend getting another wide excision done so that clear margins are obtained. That would suffice for treatment.

Results are very good after surgery.

Thanks for the reply doctor. Yesterday I visited the dermatologist, he has referred me to a surgeon for further excision. Hope everything will be fine.
