Question: my fasting blood sugar after less than 7 hours of fasting today is 292. I am on Insulin TRESIBA, 12 units at bedtime. Orally I take Januvia 100 and Glycomet 500 both after breakfast once a day. My fasting blood sugar has of late been more than 140 but not ths high. I had rice for dinner last night, which is not very unusual thoughfor me.

Hello,

I can understand your concern. As your fasting sugar level is 292 which is much higher. Ideally fasting should be for eight hours. What is your post pradial blood sugar level and HbA1c value? Kindly do if not done earlier. Let me know whether do you have any symptoms of hyperglycemia like dry mouth, thirst, frequent urination? Kindly follow up with detailed history.

At 292 sugar level, as you are taking tab. Januvia 100 mg and tab. Glycomet 500 mg in morning only. You can take tab. Glycomet 500 mg at night. I'd suggest you to do strictly sugar level monitoring at least thrice a day to avoid hypoglycemia. We can titrate inj. Tresiba dosage as per your sugar level and HbA1c value. So I'd suggest you to consult with your diabetologist for dosage adjustment and proper examination. Plan your diet as per diet chart. Do Serum creatinine and fundoscopy examination if not done earlier. I think this will clarify your concern.
