Question: Hi Doc,

From past two months i have having this mild left chest pain(on and off)- Sometimes feels like gas pain and sometimes like muscle pain.

With chest, the pain starts from my left neck, back left shoulder blade shoulder and goes down till my hand and fingers. Sometimes its tingling or numb feeling in the hands, specially in the morning.

Back shoulder blade, and hand pain increases while resting or sleeping and no pain during regular activities.

Also, when i sleep on my chest down, it hurts and feels heaviness.(Doc said may be due to inflammation).

Please advise, this is giving me serious anxiety problems. Doctors said, it must be some muscle spasm which would heal down but its been two months and no relief. infant the pain in the left arm increased now.

Chest pain is not all the time but on and off now.

Brief Answer:
Possibly cervical disc pressing on the nerve

Detailed Answer:
Hi XXXXXXX
I am Dr Gopal Goel, Orthopedic Surgeon.
I have read the details given by you and the fact that TMT is positive.
Still , with your clinical picture , I think that you are suffering from cervical disc prolapse at C4-5, C5-6.
I would advise you to get one MRI of neck to see for cervical disc, and also blood test for Vitamin B12 and D3,
In the meantime start mild pain killers , muscle relaxant and a combination of methycobalamine and pregabalin.
Let us also know the reports of MRI and Blood Tests.
Thanks for the question. Please feel free to ask any other question.

Thanks for reply. So TMT being positive is not a concern?

I had done my Blood test for Vitamin D and B12

The results are

25-OH VITAMIN D (TOTAL): 17.88
VITAMIN B-12: 347

I will be getting my C-Spine MRI done soon and will update you on the same.

Brief Answer:
Results are on lower side

Detailed Answer:
Hi
In my opinion both S Vit D3 and B12 are on lower side and should be treated.
TMT being positive may be of concern but this pain does not appear to be due to cardiac problem. A Cardiologist is the best person to talk about TMT.
Let us wait for the MRI report also.
Thanks
