my Dad is 71 and is being a sugar patient since last 40 years
his urea level in 58 and creatinine is 2

Hi,
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.
Based on your query, my opinion is as follows:

a. Secondary to diabetes, mild renal damage is present leading to rise in urea and creatinine levels.
b. Glomerular filtration rate or gfr measurement is necessary to evaluate the stage of chronic kidney disease and to check for progression in future.
c. At present, lifestyle modifications along with diabetes control is necessary for further deterioration.
You can:
1. Control diabetes.
2. Control hypertension, if any. Particularly renal protective agents like ACE inhibitors can be used.
3. Low salt diet and calorie restriction.
4. Avoid medications or procedures, which could damage kidney like most painkillers.
5. Every 6 months followup with hemogram (for evaluating anemia), renal function tests, and ultrasound.
6. Also need to look for a complete cardiac workup, if not done.

Still early stages. Based on GFR and stage of kidney disease, management is possible. Right now, following lifestyle modifications and diabetes control is necessary.
