Question: 63 yo female have complex morphology tear posterior horn medial meniscus. after 10 physical therapy sessions still have pain in area, especially after working (mostly standing, walking) avoiding stairs, squatting, kneeling should i continue therapy or consider any type of surgery

Usually meniscus tear at your age need not required any specific therapy for meniscus repair. You can continue physiotherapy. If the pain is unbearable you can consider total knee replacement.

once torn, is the meniscus likely to continue to tear? eventually to a point that some kind of surgery is the only outcome? Is arthoscopic surgery a means to repair or stop the progress of further tearing?

Hi,

Arthroscopy would be meaningless if you have arthritis in your knee as well. Meniscus may not heal spontaneously due to the lower blood supply so unless it is repaired surgically it may heal.

there is arthritis in the knee. unknown if tear is in red zone or white zone. did you mean to say that it may not heal spontaneously due to the lower blood supply, unless it is repaired surgically it may NOT? heal.

Hi,

Red zone tears heal so meniscal repair helps. Other zone tears don't heal so removal of the torn portion is done. In knee arthritis, knee replacement usually helps.
