Question: I am a 67 yr old female. I was diagnosed with pulmonary hypertension 4 yrs ago. my pulmonary doctor feels this is a results of sleep apnea. I am on a CPAP machine hs.

I was diagnosed with pulmonary hypertension four years ago. My pulmonary doctor feels it is a result of sleep apnea which was diagnosed after my pulmonary hypertension. I am on a CPAP setting of ten at hs.
I still sat in the high nineties. I have not gotten any worse since my diagnoses. I do get SOB with exertion some days worse than others. My doctor says my pulmonary hypertension is moderate. What is my prognosis?

Brief Answer:
Prognosis is good if you follow lifestyle modifications and CPAP machine.

Detailed Answer:
Thanks for your question on Healthcare Magic.
I can understand your concern.
Sleep apnea can lead to secondary Pulmonary hypertension (PH).
Secondary PH needs optimal treatment of primary cause (sleep apnea in your case).
So if you follow lifestyle modifications and CPAP use properly, your PH will not worse and you hence prognosis will be good.
Lifestyle modifications include
1. Weight reduction and strict dieting
2. Avoid excessive salt and oil in diet. Limit your water intake to less than 2 liters per day.
3. Quit smoking and alcohol if you have these habits.
4. Regularly check 2d echo every six monthly to know severity of PH. Also get done sleep study annually and titrate CPAP machine accordingly.
If you follow these, your PH will not worse.
Hope I have solved your query. I will be happy to help you further. Wish you good health. Thanks.

I have had a heart XXXXXXX four years ago. Should I have another? I am concerned about right sided heart failure. The heart XXXXXXX I had done previously was of the left side.

Brief Answer:
No need to go for right heart catheterization every time.

Detailed Answer:
Thanks for your follow up question on Healthcare Magic.
I can understand your concern.
Right heart catheterization is the most accurate way to diagnose PH.
But it is not necessary to undergo right heart catheterization every time.
2d echo is also a good tool to monitor PH.
And yes, with worsening PH there is risk of right sided heart failure.
But if your PH is noted worsening then no need to worry for right sided heart failure.
Hope I have solved your query. I will be happy to help you further. Wish you good health. Thanks.
