Question: I am trying to ascertain dosage of lisinopril to substitute for trandolopril 2mg i take twice a day. Formulary requests change for dollar factor i am

Normal bp and labs with meds. I also take hytrin 10mg hs. Lasix 20 bid losarten 100/hctz25 once daily. Verapamil er 240 hs. My bp under fairly good control. United health formulary wants me to change verapamil out for cheaper ace inhibitor. I wonder if 10mg daily is adequate substitute or 5 mg bid. Btw i am retired med doctor

Hi,

Thanks for writing back,

It's a pleasure to know that you are a doctor yourself, it will be easy for you to understand how we adjust the dose.

Trandolopril has half life of 10 hours and lisinopril has half life of about 13 hours, but trandolopril is more potent, so we have to use trandolopril in lesser amounts but in BiD dose.

I would tell my patients that, When you are going to switch to lisinopril I would suggest you to start with 5mg BD , taking them 12 hours apart.

Keep checking your blood pressure thrice a day if it stays well well under limits then you won't have to worry about anything,

Here I want to mention that you are taking diuretic so keep a good check at your BP in starting days like, specially in first two hours of tablet if blood pressure stays above 90/50 then you can continue, with time you will learn about your body habit , how it responds to this particular medicine. If you go to hypotension you might want to reduce dose of lisinopril, to 2.5mg but at the moment you can start with a 5mg BiD.

This was my opinion how i would start Zestril(lisinopril) to my patient, taking them off trandolopril.

You can dicuss with your doctor and start the dose. I believe there shouldn't be any big issues in this. Only thing you have to take care about is hypotension, and being a doctor yourself i believe you can well take care of it.
