Question: hii doctor unable to open mouth fully due to chewing tobacco how to get immediate relief or any surgery please suggest ??

Hi,
Welcome to healthcare magic.
This could be due to oral submucosal fibrosis.
Prolonged local irritation caused by chewing tobacco could gradually lead to fibrosis of submucosa of oral cavity. This fibrous bands limit the mouth opening.
I recommend the following.
1) Avoidance of tobacco, betel but and other tobacco based preparations.
2) Local injections with steroid and hylase are useful in the initial stages.
3) Antioxidant supplements.
4) Local anesthetic based gels to apply in the oral cavity, to relieve burning sensation in oral cavity that is usually present with this condition.
5) Avoid spicy foods.
6) For advanced stages, there are surgeries to get adequate mouth opening.

Surgery is indicated when mouth opening is severely limited. Submucosal resection of fibrotic bands and replacement with split thickness skin graft, buccal fatpad, tongue flap is used to improve mouth opening.
