What Does Ultrasound Showing Dilation Of Calyx Regarding Abdominal Pain Indicate?

Question: Hi, I have had an ultrasound after an abdominal pain, it showed mild prominent calyces in right kidney. Doctor gave me a few medicines suspecting stone for 5 days.After that I have had no problems.It happened in April this year. I had a 1-2 drop of blood come out 6-7 month back from urinary tract.That was the symptom due to which doctor felt it could possibility of a stone.However every thing has been normal since then.Except this pain in abdomen due to which I had ultrasound.

However I am experiencing some kind of sensation at the point where I have experienced pain. The sensation is not very prominent so as to I go and tell as what exactly is happening it happens some times not regularly for 30-40 seconds or less than that. The symptom I am not getting as how define it.

In my family my sister has had intestinal tuberculosis and we came to know at a very very late stage when we had to admit her to an emergency situation where a lot of poisonous fluid was coming out of her intestines which doctor came to know when they did some tests by putting injections and taking fluid out of her body she had undergone for 1 year hospitalisation where the intestine was cut in 2 parts and we were giving liquid food as told by doctors in hospital through a funnel and they had asked to monitor the amount given from funnel and then from another funnel they giving the food which was filtered as result of feed from first part of intestines. I do not know what is medical term for this so described this as this was the case in our family 5 years back and for 1 year no one could understand with symptoms as what exactly happened it was only when at the last moment when all treatment had failed and a lot of medical problem occurred doctor admitted and came to know all this. I thought mentioning this fact is important in describing my case.

I am a bit fearful after having gone through this kind of experience in my sisters case so I want to get some medical tests done so that if I approach doctor in my locality I can tell him some thing. As of now I am unclear as what is happening my abdomen specially at the right side. Internet search showed me if a person is feeling tired all day and not active then that is a symptom for some problem in liver. I do feel tired but not sure if it is related to my problem or stress.

I have had a past 12 years back where due to a lot of gas I got a pain chest, then a few days later heavy fever with pain in joints (at the age of 19-20 that time), I was a student that time preparing for some engineering exams which was very very stressful followed by a schedule which was very disturbed often I was in coaching or classes where I could not go to bowel motions. Forced under those conditions I had no option but to wait for class to finish. At a later date it resulted in passing of white mucus with my bowel movements.

I am right now preparing for another exam and drink tea/coffee a lot. Since at home so do not stop bowel motions. But I feel lack of concentration/tiredness and a heavy abdomen.Recently I am experiencing after I go to toilet and I feel that I am done with immediately in 2-3 minutes again I have to rush to a toilet seat and a lot of liquid water kind of thing comes out followed by motion.
I had a lot of coughing 2 months back when I had gone to doctor, however I do not have it now.I thought it to be some kind of allergy because my room is at the topmost floor and dust might be there (I cleaned the room) but recently came to know it is also a symptom in liver/stomach problems hence mentioned here.

I have currently had following tests
1) Liver Function test Results normal
2) PT,PC results normal
3) Blood Sugar Random , results normal
4) Creatinine , results normal
5) UREA ,normal
6) Ultrasound, mild prominent calcyes in right kidney.
7) Liver,Gall Bladder, CBD, Pancreas,Prtal Vein,Splenic Vein, spleno-portal axis,
Spleen, Urinary bladder, Prostate etc are mentioned in ultrasound report and things seems here in report normal.

I have recently started doing yoga and I am experiencing a lot of gas passing.Also upon drinking milk I experience gas coming out of body since the occurence upon drinking milk is slightly more than when not drinking milk so can not say if it is lactose intolerance kind of case.Never had any test for lactose intolerance.(Never thought of it just recently Google to mention things here ).
I do not smoke, no alcohol. I just drink tea a lot.
I am a bit afraid of any medical complication developing at a later stage as I mentioned my sisters case so I am being cautious here and if any tests I should take let me know.I will be happy if things are normal. I do feel something in right part of intestines/abdomen at the point where I had pain but I am unable to tell what exactly is it (it is not pain).So not sure what to tell to doctor.

I do understand your concern and apprehensions. First and foremost let me reassure you, there is definitely no cause for concern.

Ultrasound report is not that worrisome. Dilatation of calyx is likely to be due to blockade by stone, leading to increased back pressure and dilatation.

From your description given , following could be the reasons for your symptoms

1. Acid peptic disease: Excessive consumption of tea, stress(exam related) and anxiety are causative factors. There is pain or discomfort in upper abdomen, intake of certain food may aggravate the pain.
2. Reduced intake of fiber diet
3. Stress
4. Anxiety.

Its unlikely to be anything serious( If there was any gross pathology it would have been definitely been picked up by Ultrasound. Also all your blood parameters are normal, except for slight increase in Alkaline phospahtase. Even if there was any Liver disease it would have been picked up by Ultrasound. Acid peptic disease requires Endoscopy for diagnosis, but clinical symptoms suggest the possibility of it. You are young and there are no associated risk factor like alcohol for you to get Liver disease.

I would suggest you the following

1. Decrease the quantity of tea consumed: I know it helps especially during exams( Majority of doctors would be heavy tea/coffee drinkers because of professional exams). Decreasing the consumption will be helpful
2. Regular intake of small quantity of meals: 5-6 divided meals per day
3. Follow rule of 1: 1 glass of water 1 hour before meals and 1 glass of water 1 hour after meals. This will ensure adequate intake of water and avoid consuming water during meals
4. Start brisk walking
5. Antacids: Proton pump inhibitors like Pantoprozole + Domperidone ( Brand name Pantop D ) taken initially twice a day for 7 days and then once a day early morning on empty stomach.
6. Liquid antacid: Simethicone containing antacid will be helpful for your gas problem.

If the symptoms persists even after 15 days, I would suggest you following

1. Thorough re-evaluation by your Physician
2. Repeat Ultrasound
3. Repeat LFT
4. Upper Gastro-intestinal Endoscopy especially if you have upper abdominal discomfort/pain
