What Does This Urine Test Report Indicate?

Question: Hi
I am 56 and have IGA nephropathy for the past 7 years which is being treated with Cardace 10 mg, Minipress XL 2.5 mg and Lipitor 10 mg. My latest spot urine test showed Erythrocytes + in the urine, protein + , and RBCs 2-3 hpf. A blood peripheral smear examination was normal for RBC, WBC, Hemoparasite and Immature Cells but stated that Platelets were mildly decreased and I had mild thrombocytopenia. A subsequent blood platelet count showed that the mumber was 100,000 thousand/cu.mm. In past tests in the last 7 years the platelet count has varied from 128,000 to 153,000.
My questions are as follows:
- Is the platelet count a cause for worry at 100,000 thousand/cu.mm ? It appears that my platelet count has generally been below the threshold but never as low as 100,000. There are no symptoms of bleeding or fatigue.
- Do I need to take medication based on the current platelet count ? Since I have IGA i have been advised no to take NSAIDS.
- Would taking calcium tablets help to improve the platelet count? Aee there any other foods I can take to improve the count ?
- When should I repeat the platelet count test?
Grateful for your help with these queries as I am a bit anxious.

The platelet counts show that you have had low platelets over a long period of time.

Use of NSAIDs over a long period of time is a possible cause of low platelets.

However, platelets count of one lakh and above does not have a deleterious effect like bleeding.
So, you need not worry about complications like bleeding.

Calcium supplements will not correct the platelet count so there is no requirement to take calcium just for platelet issue.

You can repeat platelet count repeated at a gap of six months.

As long as there is mild thrombocytopenia (low platelet count), you are safe from bleeding and body tolerates it well.

1. Is there any changes I can make to my diet to improve the platelet count?

2. At what level of platelet count will I need to seek more detailed medical attention? You have mentioned that platelets count of one lakh and above does not have a deleterious effect like bleeding -at what level does the platelet count have an effect of bleeding ?

3. Does the platelet count tend to vary a lot i.e. does the count vary a lot every time a test is done ? My last platelet count was in Feb 15 and the count was 150,000. Between Feb15 and now May17 it has dropped to from 150,000 to 100,000 -hence my concern at the sharp drop in the count.

Platelet count below 50,000/microlitre is associated with risk of bleeding.

1. Diet does not affect the platelet count.
2. Platelets below 50,000/ microlitre are associated with risk of bleeding.
3. Variation in platelet count can occur when done at a gap of one month.

You may repeat platelet count after a month.
If the platelet count remains between 1 lakh to 1.5 lakh, there is nothing much to worry.
