Brief Answer:
Urinary tract infection (UTI)

Detailed Answer:
You are having acute urinary tract infection (UTI) as suggested by pyuria (urine microscopy showing 25-30 pus cells/hpf and occult blood).

The commonest organism of UTI is E.coli-usually susceptible to Nitrofurantoin (NFT) but there may be other organism resistant to NFT.

Most fluoroquinolones (commonly used for UTI -ofloxacin)are highly effective for short-course therapy for uncomplicated cystitis.

Repeat your urine routine examination.
If Pus cells count has not diminished by now, after NFT given for more than 72 hours, addition of ofloxacin empirically would be justified.

Discuss with your doctor.
