What Does This Ultrasound Report During Pregnancy Indicate?
Question: Hello Doctor,
My wife has thallasemia minor and placenta grade : Grannum III at 34W+3d. And the US scan has the below comment :"The placenta is on the posterior wall, the low margin is 2.7 cms away from the internal OS."
The fetal growth, amniotic fluid(AFI - 13.8) and doppler are normal.And the baby weighs 2246 grams.
What are chances of my wife to have a normal delivery ? Will there be a chance of an early delivery ? Are there any risks associated ?

Thanks for the query to HCM for an opinion,
The scan is overall normal and corresponding to the period of gestation. A posterior placenta is safe and the only slight concern being the distance of lower margin of placenta being 2.7 cm away from internal os. The cut off is 2.5 cm at 32-34 weeks.So theoretically she is safe and doesn't have any placenta previa.
Baby weight is also corresponding to the gestation and AFI is more than 10 cm which is good.
IF your wife has a roomy outlet and has medium height , above 5 ft 3 inches , then the potential chances of cephalopelvic disproportion is unlikely which is one indication for c-section. Though the pelvic assessment is always done on labour table once the woman goes in labour and no assessment is done prior to avoid any risk of rupture of membrane or initiating premature labour.
If the report says that the presentation is cephalic , i.e the head is down towards pelvis then the chances of normal progression to vaginal delivery is likely if all goes well and should be without complications. If ever the is an accidental bleed due to lower margin of placenta during the first stage of labour and formation of lower segment, then she may be taken up for c-section in interest of the mother and the baby.
