What Does This Ultrasound For Kidneys Indicate?

Question: Hello I am a 46 yr old woman who has had rad hyst with chemo radiation. Three years ago and also pulmonary embolism. Now I have had ultrasound for kidneys due to lack of voiding. Tests show right kidney cortical irregularity and scarring and loss of parenchymal thickness. Cretinine last taken 4 months ago was ok at 97 and eGFR 60 and urea 8. Bladder fine. Thankyou

I have read through your query in detail.
Having worked in a cancer institute for few years and have seen many patients of cancer cervix with conditions affecting the kidneys and bladder. I guess that you have got radical hysterectomy and radiation for a possible diagnosis of cervical cancer. Presently radiation is given over a small field and it is unlikely that your radiation included the kidneys. However, it is possible that the field of radiation was over the pelvis and might have included the urinary bladder and lower end of ureters or bladder outlet. This can cause strictures in the ureters or urethra causing some amount of obstruction to the flow of urine resulting in back flow of urine to the kidneys causing irreversible kidney damage that includes scarring and thinning of kidney cortical parenchyma.

From the values provided creatinine is 97 and eGFR is 60 then it is under chronic kidney disease stage 3 A. You need to consult a nephrologist and be under constant supervision to salvage the kidney functions and slow the progression of kidney disease.

Thankyou for your input Dr Chail it is not a backflow as this was on the report stating there is no hydronephrosis, sorry I did not add this. My Gp said I should see my Gyn/oncologist to further this on. I will make an appointment today.
Also does this type of injury to the kidney cause hormone problems as I have gained 20 kg in two years on limited diet. Thankyou

Thanks for writing back with an update.

I appreciate your information and I was of the opinion that you have obstruction to flow of urine as you had mentioned ultrasound for kidneys due to lack of voiding in your previous message.

There are however other causes of kidney cortical parenchymal thinning other than obstruction to flow of urine. This can also cause kidney scarring and decline in kidney functions as seen by slightly increased serum creatinine and decreased eGFR.

There is no direct relation between an injury to the kidneys and increase in weight due to hormone problems. On the other hand, obesity is linked to diabetes and hypertension, both of which can cause kidney diseases if not controlled.

Your kidney condition needs to be investigated and if required, a biopsy will have to be done to know the changes in kidney cells which can confirm the exact condition. You might also need to get urinary protein excretion tests. Serum creatinine levels are to be monitored regularly.
