What Does This Semen Analysis Report Indicate?

Question: Have conducted Semen analysis and Macrophage cell is showing Present ++, but pathogens are showing nil. Rest all counts and viscosity are showing normal. Please let me know if having high macrophage cells are an issue?

Thanks for the query under HealthcareMagic . I have read it & understand your concern.
Once increased macrophages are found in semen .. more evaluation is needed to find the cause & consequences Specially if the person is suffering from sterility.. Following things need to be seen.
- Morphology of sperm is important even if the counts are normal .
- Chromosomal study needed to find any genetic cause.( DNA problems)
- To detect Anti sperm antibodies to for immune system involvement.
In your case there are many types of abnormalities seen in sperm morphology.This itself can cause increase counts of macrophages in semen.. for one of the work of white blood cells is to remove abnormal forms of sperms from semen.
The causes of abnormalities in sperm morphology can be Genetic/ exposure to toxic chemicals( like smoking, working in automotive paint shops )/ increased temperature of testis.
* I would advise to consult an expert pathologist/ sterility specialist to evaluate your problem & find best of the best solution.

We have been trying for a baby since past one year with no result..Initially the Viscosity was present +++ while macrophage cells were occasional. I was advised to take antibiotics for viscosity problem. And now viscosity is normal...So why is this sudden increase of macrophage cells now..And could this affect in conception?

Thanks for the follow up query.
Actually you had uploaded only report of spermiogram denoting about 68% abnormal sperm forms in the sample.
As per the recent information..you were given antibiotic for raised viscosity of semen. Raised viscosity could be due to chronic infection/ inflammation of epididymis of testes, sex gland dysfunction or oxidative stress.
- As there was no effect of antibiotics, you were advised spermiogram to detect the problem.We have also seen that cause of macrophages in semen is to remove the abnormal forms from semen... here one more thing happens.. activity of macrophages causes increased oxidative stress.. which in turn hampers normal activity of sperms .... Thus making a vicious circle to affect conception.
- To break this vicious circle the whole thing needs to be reassessed by more detailed tests of semen ( biochemical for determining oxidative stress / culture & sensitivity to completely exclude / find infection.& also detailed examination to pin point basic cause of sperm abnormality by an expert. ( viz genetic cause/ exposure to toxic agents/ raised temperature of testes.
- Please don't worry. Mostly all hurdles except for genetic cause are treatable.
by an expert advise& you would recover.
Self help - Simple personal measures like avoiding exposure to extreme heat /avoiding wearing tight undergarment- thus avoiding over heating of testes
- Taking antioxidants like Vitamin A, E ,C, Selenium would help.

I have uploaded the motility report also right now... I was advised to take Vitamin E for Viscosity problem.. I was also asked to do semen culture and the report says no organisms detected and no pathogens at the end of 48 hours of incubation at 37 C.

Thanks for the follow up query with additional information.
Your reports done on the same date as the spermiogram ( 4-3-17).. the sperm motility report along with details of general semen analysis..
- Rules out possibility of antibiotic resistant bacteria/ chances of infection as a cause of increased viscosity.
- The sperm counts are within normal limits,
- The sperm motility also almost reaches normal finding ( 45% of sperms showing total forward progressive movement )
- The spermiogram shows 68 % abnormal forms on the same date. Though there is no mention of normal sperm form percentage..we can draw an inference ( to be confirmed by exact figures ) that remaining 32% are normally shaped sperms.... which gives a ray of hope according to ' Kruger strict criteria' .. which says at least 15 % of sperms to be perfectly normal shaped is acceptable!
* Still the fact remains that more the number of abnormal sperms are.. more is difficulty in conception.
Conception is a complex process. So normality of health of both the partners & care of all minutes is needful for positive result. So I would finally suggest-
- Investigations to find the cause of oxidative stress on sperms & try to overcome it with specific advise of an expert & self care.
- Use of antioxidants, healthy life style to have optimum weight , diet & exercises along with a positive psychology would bless you to full fill your desire! Wish you all the best !
