What Does This Lipid Profile Test Report Indicate?

Question: i just had my TMT done and also echo 2D. Echo was fine, TMT is positive for RMI. I have had a angiography also done in Sep 2012, LAD. My Lipid profile is around 238 but triglycerides at 142. The doctor is insisting on doing again angiography. What are my options??
I am non-diabetic, non smoker.

the TMT result also stated Fair Effort Tolerance MET- 7.5 @ 109% of THR

Hi,

There are no details about the previous angiogram or medical history. What is your blood pressure. Do you have family history?

Since the lipid profile is not up to the mark with higher cholesterol levels there is a need for checking the patency of the blood vessels supplying the heart. You can go in for coronory angiography. If you do not want intervention technique there are other non invasive techniques like CT Angio / MR Angio gram, where they inject the contrast and put you in the machine which flows through. It helps detect the patency. 95% of the times there is a block but how much we do not know.
The disadvantage here is if there is any block you have to undergo invasive technique to lyse the block. That is why your doctor may not want a second procedure he insists on angio.
With the brief info you provided this is my opinion.

Thanks for your opinion. I had my angio done on 9th May. My old stent in LAD is still patent. However, RCA has a 100% blockage. There are collaterals and retrograding(?) collaterals from LAD/LCX. Thrice opening of the blockage was attempted but they could not get through. Finally the procedure was abandoned.
what do you advise and what are my further options?? Any risk of immediate nature?? any medication or methodology to open up the blockage?? BP is around 90/120 with medication metroprolol 25mg BD and Losartan 2.5mg OD.

Hi,

Although collaterals and retrogradings are good sign and they will perfuse the remaining heart muscles, they viability is doubtful long term.
If the opening attempts were not successful I think they will consider a CABG (Bypass graft) procedure. This time they can consider an arterial graft rather than a venous one.
I think my expertise is not sufficient beyond this context. I do patient education in cardiology but do not know the protocols and standards followed after unsuccessful attempts.
