What Does This Echocardiogram Report Indicate?

Question: hi,
i'm a 55 y.o. male fit and active though a diabetic (well controlled) since 10 yrs.
I had 2 stents put in around 10 yrs ago.
A recent angigram showed a minor block in OM2 and was advised a stenting.
I do 1 hr weight training 5- 6 days a week and walk for 1 hr 6-7 days a week
Today my 2 D echo had the following remarks:
1. LV RWMA + HYPOKINETIC LCX TERRITORY
2. FAIR LV SYSTOLIC FUNCTION (EF AROUND 15%)
3. GRADE II DIASTOLIC DYSFUNCTION OF LV ( LV IS DILATED)

What is the general conditon of my heart ?
On a scale of 1 - 10 where 1 is a normal heart and 10 is a heart that required replacement what is the ranking you would give to my heart ?

Hello ,
I am Dr. Mody and I would be addressing your concern .
The damage to the heart may be old heart attack ( minor - not noticed ) .
However 15 is not the best EF to have ,
There is no way I can objectively rate your heart on a scale for 1 to 10 ,However for understanding purpose let's say it 7 or so . A lot of it is reversible if we can find the cause and treat it .

Coming to your query?

Hypo kinetic , Lcx territory can be explained because of the Om2 lesion particularly if it's a large vessel and hence treating the same would help recover some lost heart function . However to objectively know how much reversible myocardium is available. A test called PET scan pr stress thallium whichever is available should be considered .

If there is viable myocardium there , Angioplasty will surely help .
Do take the medicine prescribed and it too would reverse the EF to better values over the period of time .

A comprehensive advice would take into account a lot of information about the medicines you take , your symptoms , progression over years .

You may upload the reports for advice on that part.

Dear Dr Mody,

I am attaching the first page of the report where it shows LVEF as 55%.
In what way is it different from 15% LV Systolic Function stated in page2 ?
I am also attaching a recent NMR Stress Thalium report.(3 months old)
I can walk briskly for 1 hr (around 6 kms) without discomfort or hyperventilating.
I also climbed 10 flights of stairs of an apartment building earlier this year just as a self test and was able to do it without any discomfort.
I can send the CD's or contents of the angiogram or Stress Thalium Test if you want.

There is some printing error in the Echo report .Do confirm it with Hospita

Hello ,
As Stress thallium does show reversible ischaemia in the territory of OM2 , Do opening up the vessel would surely benefit you in some way

Now coming to Echo
1) The report needs to be reviewed. So do contact your again . There is some printing error .
Ef of 55 is near normal .
While EF of 15 Is really reduced function , Both cannot happen togather . Also he has mentioned systolic function to be fair which does not correlate with 15 % .
Again 3 months before the EF was 32 % as per NMR, So again that doesn't fit with both the values .
In 3 months there cannot be such drastic change in the EF parameter .
A review Echocardiography by a XXXXXXX doctor would help clear the missing understanding and help us guide the management.

Dear Dr Mody,

At that time (3 months ago) I myself got a doubt about the EF of 32%.
I researched about it in the internet and came to know that such depressed EF readings are common in NMR so I did an 2D echo the next day and the results were 48% which are more consistent with the latest report. The improvement can be attributed to Arjuna and Hawthorn which I'm taking since the last 3 months.
I am planning to stent the OM2 by the third week of this month.
The detailed NMR showed a reversibility of around 92% which I hope is true.
I'm taking Arjuna + Hawthorn as various literature reports say that since these are powerful adaptogens and very cardiotonic they do the function of thinning blood as well as increase cardiac muscle strength, lower BP and lipids and regulate the rhythm all without adverse effects; therefore I prefer it in comparison to 3 or 4 different synthetic allopathic formulations.
I am attaching the 2D echo done at that time at Apollo Hospitals alongwith images for your comments.

I would enlist my observations
1) Yes NMR is not the best test for EF and the aim was to know viability. More than 50 % reversible would form a case of revascularisation . So with 92 % it should surely help

2) All the best for a planned angioplasty ,It would mostly be an uneventful procedure .

3) I am not the qualified person to guide you on anjuna and hawthorn.
As a man of evidence I would suggest that you don't stop the evidence based proven medicine which would help . With that you can take these complementary medicine.
Just be cautious as taking extra blood thinners may increase the chances of bleeding

4) The appollo Echo report is not attached . However it dose fit into clinical picture you describe .

Dear Dr Mody,

Thank you for your detailed mail.
I will surely heed your advice to take prescription medicine alongwith Arjuna ( it has been proven to work - evidence based scientific trials ) and there are trials done to show that Arjuna can be concommitanly be used with allopathic medicines without interference and actually complement the prescription medicines.
I am attaching the Apollo echo reports again.
I would like your opinion on the other observations in the report apart from EF such as dilated atrium and whether they should be of some concern.

Brief Answer:
The dilated LA is secondary to mild decrease in heart pumping .

Detailed Answer:
Hello ,
LA is the left upper chamber of the heart which receives blood from the lungs and passes it to the Left ventricle which pumps it to the body . In the Echo report the La is mildly enlarged .

However it will not make any changes in the treatment plan at present. We would just keep on monitoring with a yearly Echocardiography .
It can be explained because of impaired relaxation of the Left ventricle ( Due to long tern hypertension or mild decrease in pumping ) .

You may continue the supplementary medication , however just let your doctor know the same so he can adjust the allopathic medication to prevent any drug interactions.
