What Does This Abdominal Ultrasound Report Indicate?

default
Posted on Mon, 23 Nov 2015
Question: Hi Sir
Recently I am facing full stomach after meals. Also I find it hard to breathe. This ends with lots of burps. I underwent a USG abdomen, everything was normal except grade 2 fatty liver. Also I feel burning sensation in chest.

Is my ultrasound report sufficient to rule out any major issue. What should I do to prevent full stomach feeling after meals. Is breathless feeling a concern?

Thanks

Brief Answer:
complete lipid profile and liver function test

Detailed Answer:
dear,
welcome to HCM,
We can understand the discomfort you are through.

After understanding your complaints, i am suspecting dysfunction of gall bladder and liver.
We have already established a Grade II fatty liver condition. Gall stones can also present with similar picture, however a normal gall bladder on ultrasonography has ruled out any gall bladder related abnormality. I presume(as you have not mentioned) that there is no associated pain in abdomen, no nausea, no vomiting and no fever, which rules out any possible infective conditions of hepatitis, cholecystitis, etc.

The symptoms of heart burn(indigestion), full stomach, breathing difficulty, etc are explainable with fatty liver condition. Therefore, now we look forward to manage the condition of fatty liver. However, before management we need to asses the function of liver for which i would suggest few blood investigations such as Compete Lipid profile, Liver function Test, Complete blood counts, and random and fasting blood levels. It will help us to know the cholesterol status, liver enzymes, which are commonly deranged in the case of fatty liver condition.
Secondly, i would need to know
history of obesity, hypercholesterolaemia, diabetes in your family?
History of obesity in you? kindly mention your height and weight here?
Do you consume alcohol? quantity?
Any history of rapid weight loss?
How is your lifestyle? sedentary, moderate activity? do you exercise daily?

For now i would suggest, there is no major concern to be worried as fatty liver being a reversible condition can be controlled and cure by proper modification in diet and lifestyle. Eat small meals, include vegetables and fruits in diet.

Kindly revert back with the information regarding above questions for more suggestions.
Thanks for asking.

Brief Answer:
increased for propensity for triglyceridaemia

Detailed Answer:
Thanks for the reply.
Your reports indicate high levels of Triglycerides and low HDL. The rest of values are within the normal range.
It can be said that you have a propensity for hypertriglyceridaemia, a condition in which Triglyceride levels are high. Although the levels are not very high, however it poses risk for pancreatitis and cardiovascular complication if left uncontrolled.
It is therefore it is suggested to restrict the the consumption of saturated fat, and increase the intake of fibres and good cholesterols.
The fatty liver can be explained by this. A further specific investigation of lipoprotein levels evaluation can provide specific results.
There is an indication of fibrates, as they would reduce the absorption of low density fats.

My final suggestions
1) Consider the use of fibrates after consulting with a cardiologist or dietician(comprehensive diet plan for restricting fat diet), increase proteins diet.
2) Make some lifestyle modification such as moderate exercise, etc.
3) Look out for the signs of pancreatitis such as severe abdominal pain, fever, vomiting, etc.

Hope to have satisfied your query.
Stay healthy.

Dr Pankaj Borade
