Question: My brother in law had gone for general check up, in ECG, Doctor identified some problem. So we want 2nd opinion from the XXXXXXX and from the specialist

Hi,

I was going through your ECG report, and I can say as following:
- sinusal rhythm (normal)
- elevation of ST segment with poor R progression in V1-V4
- depression of ST with negative T in lead II, III and aVF
- negative T in V5-V6 and lead I

All these data are indicative for ischemic heart disease (coronary arteries blockage).

If you were my patient, I'd ask to run the following examinations for a proper evaluation:
- echocardiograme
- stress echocardiograme
- cardiac angio CT-scan
- and angiography

These tests are necessary to determine the right pathology and the right treatment strategy.

Thanks for the answer, attached is the 2d echo scan report of the person and we have gone thru the angiography test which is now in compact disc version reporting that a 100 %blockage of LAD and 80%,80%blockage of 2 right arteries. As the echo report is showing 30%EF what is the best possible treatment we should consider to plan now
Is it necessary to go for bypass surgery immediately or stenting, suggest a best possible action.?

Hi back,

I read the echo report very carefully but to give you a more accurate opinion I would like to have the angiography results.

However from what you are saying your brother in low has a two vessels disease (blockage in LAD and RCA) and severe calcified aortic stenosis (on Echo report). In such a case I would suggest bypass surgery together with Aortic valve replacement as the best thing to do.

Coronary stent will treat only coronary blockage, but it will not deal with aortic stenosis. The only action that treats both (Coronary blockage and aortic stenosis) is cardiac surgery.

Yeah these are the results of the angiography and 2d echo tests..is there any possibility for bypass now as EF is only 36. Shall we go to surgery for both arteries and valve at a time now. Is there any chances to improve EF of the heart

Hi back,

I check the angiography results and the second Echography results. I can confirm that your brother in low has a severe triple vessels disease (triple coronary blockage and a damaged heart with a reduced EF of 36% as a result of it) and mild to severe aortic stenosis.

If he was my patient i would recommend cardiac surgery (bypass surgery together with aortic valve replacement) as soon as possible (within days, because it needs 1-3 days of preparations before this type of surgery).

There's no chance to improve his EF, the only chance to prevent the things going worse (i.e. EF to be reduced further) or to prevent another heart attack is cardiac surgery. This is why it is recommended to do cardiac surgery as soon as possible.
