What Does The Urnie Analysis Report Indicate?

Question: Hi Doctor,

My Son, who is just complete 2 months 15 days. At the very beginning (after 15 days of DOB) he had been crying at the time of passing urine. That time our paediatrics was suggested to do some test and on the basis of test report he gave one drop for 10 days. After completion of this medicine course (after 10 days), we have done again the same test.
In test report we found the following results.

Urine Analysis
Appearance: Faintly Turbid
Colour: Straw
Reaction: Acid.
Specific Gravity: 1.010
Albumin: Nil
Glucose: Nil
.........And all reaming are Nils.
PUS CELLS (1-2/HPF) And A Few Epithelial cells presents.
Culture       Put up

Urine Culture : Citrobacter koseri (>100,000 CFU/ml)
------------------------------------------------------------------------------------------
There have one more report of urine culture for Citrobacter koseri, and in this report maximum places have mentioned ‘R’ Resistant, one place has ‘I’ Intermediate and reaming place have ‘S’ Sensitive.

For example
Cefepime 32 msg/ml R.
Like that way there have multiple rows.

After checking this report, our paediatrics refers to some nephrologists. So, it would be very helpful for me if any one of you give the answers for the following questions.
1.     Is this report showing really any big problem?
2.      If yes, is it fully curable?
3.      Please help me to understand, where the exact problem is. Is the problem of Resistant of antibiotic or UTI infection or Urine Culture?
4.     What is the normal count of urine culture?
5.     What is the normal count of PUS Cells?
6.     Citrobacter koseri is any bacteria?
7.     What is the meaning of Resistant in doctor terminology?

I know, I have asked multiple questions. But I think you will be able to understand that, I have asked the entire question for my 2 months 15 days baby. If there have any problem in report, then please guide me how I will fully cure my baby.

Please Note: Now he does all activity normally. He doesn't cry at the time of passing urine. He takes the Brest feed in proper way. He passes the urine and stool multiple times.

Please help me... I will wait for your response... Thanks A Lot in advance !!!

It is very reassuring that baby is passing urine normally, playful and is having no fever.
If the following criteria are fulfilled , its unlikely to be cause for concern

1. Urine stream is good
2. Urine is clear
3. Baby is gaining weight adequately
4. Absence of fever

In the absence of significant pus cells( pus cells are dead White blood cells which are marker of infection) its unlikely to be Urinary tract infection. In a baby which is not circumcised( the foreskin is intact), normally the prepuce is very much adherent to glans and cannot be pulled back. There leads to accumulation of whitish secretion inside the prepuce. This could lead to local growth of bacteria even Citrobacter Koseri.
Citrobacter koseri can be normal flora of prepuce and skin in the perineum(skin surrounding genitals and anus). When Urine sample is collected, the urine can get contaminated with these bacteria, hence causing growth of such bacteria in Urine culture. So presence of this bacteria is culture has to be interpreted with caution.

When any bacteria is grown in culture , also antibiotic sensitivity is checked. S denotes : Given bacteria is sensitive to the said antibiotic and the antibiotic will be effective in treatment.
R denotes Resistant: The bacteria is not susceptible to the said antibiotic and hence the antibiotic will not be useful in treatment

I denotes Intermediate Sensitivity: Mid way between S and R

It acts as a guide to use which antibiotic should be used in treatment.


I routinely encounter many such babies presenting with such complaints.
If the Urine shows no pus cells and Ultrasound Abdomen is normal, there is no need for further intervention. I would suggest you to get Ultrasound abdomen ( for Kidney, Ureter and bladder) . You can do it as a routine test after appointment.
