Question: Request for specialist on Infertility treatment
Dear Sir,
     I am 34 year old married women. I am also hypothyroid and taking medicine Thyroxin 50 mcg since 2011. I am married since 2008. We tried for child from 2010. I conceived in May 2012, but had miscarriage in one month. DNC was done.
Since then we are trying for pregnancy but no success. I had tried IUI once, in Oct 14, but no success. Mine tests and reports are as followed:-
1.     Age- 33 Weight: 69 Height: 5 feet 3 inches
2.     Sugar (on 16/10/14)      :     Fasting: 105 mg/dl,
pp: 129 mg/dl
3.     FSH & LH EVALUATION, (ON 14/08/14):     LH: 2.94 mIU/ml
FSH: 4.03 m IU/ml
LH/FSH RATIO: 0.73
4.     Thyroid(on17/01/15) ):      T3: 149.48 ng/dl
           T4: 9.40 ug/dl
          TSH: 5.48 uIU/ml               
5.     Prolactine (on 13/01/15):      20.39 nm/ml
6.     AMH/MIS (on 17/01/15):      1.36 (LMP was on 02/01/15)
7.     HSG report is attached
8.      Monteux test ( on 13/01/15))     : is positive with 15mm ( But gynecologist not yet sure for any infection)
9.     Husband semen analysis reports attached
Request guidance on present condition and way ahead. all my reports are uploaded to my profile on this website. Whether, tuberculosis is the cause? If yes, which other further tests and treatment required? Should I go ahead with IUI or IVF?

I saw your reports. Following are my observations and advice:

1. For confirming tuberculosis you need get a test called TBPCR mRNA by doing endometrial biopsy. This is more confirmatory than all other tests. This is first step.

2. If TBPCR mRNA is positive, you can consider taking anti tuberculosis treatment.

3. Seeing your age and reports attached I think you should go ahead with IVF as soon aa possible.

sir, thanks for the reply.My gynecologist is not so sure of TB. However, I am planning to undergo TBPCR mRNA test and then proceed for IVF or tuberculosis treatment. Is the plan correct?

2nd query: How safe is TBPCR mRNA test ? how much it cost? planning to do in Apollo or Wockhardt. Will effect my health in anyway?

3rd query: How much chance do I have for pregnancy bu IVF? I had miscarriage once. So any further miscarriage chance?

Any more checks or tests? any chance of cancer or something???

I am very tensed. Require your complete guidance

Hello

1. You are right. You need to get TBPCR and then decide if TB treatment is necessary.

2. TBPCR is a safe and simple test. It costs about 5000 rupees. Which city you are going to apollo ? There is no risk to your health.

3. There is always chance of miscarriage in each pregnancy. But not necessary.

4. Your success rate should be around 60 to 70 percent.

5. There is no chance of cancer. Dont worry.

6. No more tests are required.

Thanks for your help. Few more queries.
Sir, my gynecologist checked and found that I have Endometriosis also (not understood). Will it affect me anyway? What I should do.
My AMH is low. How it’s going to affect me and fertility and how long I will be fertile. Any problem in egg formation? Do I need to preserve my egg?
If I am detected with TB and undertake the treatment, then how long should we wait for IVF?
Should we go for Second attempt for IUI or directly go for IVF?

Hello

Endometriosis has little effect if you are going for IVF.

I suggest you go directly for IVF as your AMH is low meaning your egg count is less.

Dont try IUI anymore.

If TB is detected you need to take treatment for 4 months and then IVF.

Sir about TBPCR mRNA by doing endometrial biopsy.
It will be done in Uterus tissue or with the fallopian and ovaries?
Is there any timing/days ( so many days after LMP ) for the test?

The test is not done in Goa. Any hospital in XXXXXXX and also in Goa, where the test can be done?
Can me and my my husband continue with sex and try for pregnancy? Or should we wait for the outcome of the test?
Is my husband semen report is ok?

TBPCR is done on uterine endometrial tissue in the beginning of cycle.

Your husband's semen analysis is fine.

You can keep trying meanwhile without waiting for report of test.

I had my last periods on 29 XXXXXXX 15. I have 28 days cycle. So when can I have my TBPCR test? Is it possible between 18-19 feb, bcz I will be in XXXXXXX
Sir, will be grateful, if you can refer any good IVF facility clinic in XXXXXXX Also in XXXXXXX if any?

My gynecologist has suggested to take Ovacare and Ovafem plus . I am taking Ovacare. The Ovafem plus is not available in the nearby medical shops. Should I continue with the medicines.? If yes plz give a substitute for Ovafem plus? Any DHEA or other medications required?

I have contacted Malpani Infertility Clinic, Colaba, XXXXXXX on email for the TB PCR test.. They have suggested following:
These TB PCR tests are very unreliable. Your low AMH level . Suggests you have poor ovarian reserve.
You can try the following to improve your ovarian reserve.
1.Yoga
2. Acupuncture
3. DHEA, 25mg thrice a day.
4. Take wheat germ daily
5. Coenzyme Q10 , 100 mg daily
6. Cholecalciferol, 60000 IU, once a week for 4 weeks
IVF with aggressive superovulation would be your best option as it would maximize your chances of getting pregnant quickly and help you make the most of the eggs you have left. Treatment takes about 20 days. TBPCR mRNA test is not required, and I can directly go for IVF.

I dont agree with recommedations of malpani clinic. Moreover ovacare and ovafem are not of proven value scientifically and are useless.

Hence you start with TBPCR as suggested before In February dates you mentioned.

If negative go directly for IVF with super ovulation.
