What Does The Following Kidney Function Test Report Indicate?

Question: Could you please give me an interpretation of the extent of the decline of renal function over a 3 day period (the results for Day 3 are not as detailed)?
(Day 1) pH 7.50; urea 27.0 mmol/L; creatinine 173 umol/L; estimated GFR 34; sodium 146 mmol/L; potassium 3.3 mmol/L; magnesium 0.70 mmol/L; calcium 2.47 mmol/L; corrected calcium 2.63 mmol/L; chloride 95 mmol/L and phosphate 1.01 mmol/L.
(Day 2) urea 30.9 mmol/L; creatinine 174 umol/L; estimated GFR 34; sodium 150 mmol/L; potassium 3.6 mmol/L; magnesium 0.73 mmol/L; calcium 2.35 mmol/L; corrected calcium 2.65 mmol/L; chloride 102 mmol/L and phosphate 1.12 mmol/L.
(Day 3) urea 34.0 mmol/L; creatinine 204 mmol/L; estimated GFR 28; sodium 157 mmolL; potassium 3.4 mmol/L and chloride 106 mmol/L.
Could you also tell me, if possible, precisely how bad the renal function was on Day 3?I forgot to mention that the patient also has COPD, pleural effusion and CCF. there is evidence of a hepatic dysfunction on Days 2 and 3.

The estimated GFR depends on body weight and age and renal function. Anything above 90 is considered normal. Anything less than 90 is considered as Kidney Failure.

A value of 28 is extremely low and is a relative indication for Dialysis. Whether Dialysis has to be done or not depends on patient's condition also like his Blood Pressure , His Urine Output .

But bottom like is his Kidney function is extremely bad.

Thank you Dr. Divakara for giving me the the benefit of your expertise. The last known body weight of the patient was 53.4 kg, but he probably lost some more weight after that time..
Would you say that the patient was probably in a critical condition even on Day 1, especially given the comorbidities of pneumonia, CCF, COPD and pleural effusion?
I appreciate from what you are saying that the estimated GFR is extremely important. But can much be inferred about the rate of decline of the kidneys from the other pathology results as well?
I very much appreciate getting an analysis form an expert in this field.

Thanks for follow up.

Yes obviously even on day 1 his condition seems critical but this is from his kidney point of view. Overall whether he is critical or not depends on many parameters like his Blood Pressure, Urine Output , Conscious level, Oxygen saturation on Room air.

Compared from Day 1 to Day 3 there has been no significant reduction in kidney function, but as said it depends on Urine Output , Ph, Serum Bicarbonate, Base excess .

If you can let me know these details I can tell better about his kidney and his overall status.
