What Does The Following Biopsy Results Indicate?

Question: Dr. My wife is 36 years old, for the past 1 month she had a swelling on the left side neck, our Dr initially gave some antibiotic but it didn't suppress later he recommend to do an FNAC but the result stated blood & numerous RBC'S, lymphoid cells not present, adviced for biopsy of node and we did a biopsy, mantoux test showed positive with 12mm, Biopsy results : section show lymphnodes and fatty tissue with larger areas of necrosis. There is dense and diffuse infiltrate of degenerating cell debris, nuclear dust, lymphocytes, histiocytes and scattered neutrophils. Epitheloid granuloma not present.
Diagnosis: LEFT CERVICAL LYMPHNODE, EXCISION - ACUTE NECTROTISING LYMPHADENITIS.
now my Dr said in XXXXXXX it's considers as lympnode Tuberculosis, and started with AKT4 along with Benadon the past 8+2 days initial 8 days where regular and altered she complained of high stomach pain and acidity so Dr told her take in alternative days so all together 10 day's. Now for the past 2 days when she takes akt4 (only)she had complains of fever and chills in night Temp range from 100 to 102 for which Dr prescribed only paracetamol. Now my question the Diagnosis for lymphnode TB is right or any other bacterial infection, taking of AKT4 has these side effects? Or I have to do some more test, please Advice me

I have gone through your question. I can understand your concern.
She has necrosis in his lymphnode. In XXXXXXX it can be considered as tuberculosis. It it first time so it needs AKT1 treatment.
AKT 4 is needed in defaulter cases. However it can be something else in rare cases. If you want to confirm the diagnosis then you can go for TB PCR. If the test is positive then take AKT1. Her side effects are common with anti kochs treatment. No need to worry. Just check her baseline SGPT level. Because rifampicin is hepatotoxic drug.
Consult your doctor and take treatment accordingly.

Thanks for the advice Dr, how long should the treatment with AKT4 has to be taken considering my wife case, is this a active TB or a latent one. food habits need to followed, after taking AKT4 medicine she feels so weak and Cnt do a normal life, she gets high temperature and chills, should she take a paracetamol 500mg or she can be left like that when time goes by she will regain, why I am asking is u said it's an hepatotoxic drug so paracetamol will also make her more weak, what should she follow, need advice

I have gone through your question. I can understand your concern. She should take AKT1 for 6 months. She can take paracetamol as and when required for fever. Refampicin is hepatotoxic but not a cause of big worry. Just check baseline sgpt level.
She should take high protein diet and vit b12 tablets.
