Brief Answer:
Needs to start on treatment

Detailed Answer:
Hi

I am sorry to hear about your wife's periventricular ischemic changes at such young age.

This is area of brain which is having reduced blood supply leading to changes associated with reduced blood supply.

She needs to undergo detailed evaluation for this so as to why this happened and how to prevent further progress of ischemia. If untreated this might lead to brain hemorrhage.

Common cause for periventricular ischemia is hypertension. If she has hypertension, this needs to be controlled adequately.

If not hypertension, other causes of ischemia needs to be looked at.

She needs to start on anticoagulant medicines which her doctor will prescribe her.

Let me know if you need any more clarification.
