Question: Hi My name is XXXX
a type 2 diabetic controlled with tablets and Beyetta 10, age 67 obese, past (1994) heart bypass.
my MRI says Hi grade involutional change of both cerebral Hemispheres and also Multiple foci of chronic ischaemic leokomalacia in cerebral hemispheres.
what do 2 statements mean

Detailed Answer:
The MRI report shows effects of long term poor circulation to the brain cells, which as a result have been lost due to lack of adequate blood supply. The net result is that the brain size has undergone some reduction.
Cebebral hemispheres are the two sides of the brain (cerebrum).
Involution refers to the decrease in size of brain tissue.
Foci of ischaemic leukomalacia refers to several individual spots in the brain where the white matter has undergone changes in response to inadequate circulation

Insufficient blood flow to the brain is not uncommon in people who have diabetes. Just like the arteries of your heart were affected necessitating a bypass in 1994, a similar process typically occurs in the brain too. And in many, it affects the circulation in the legs as well.
Of course, it can happen in people without diabetes too. However, the risk is highest in those with multiple other risk factors such :
High blood pressure
High cholesterol
Smoking

It is advisable to see a Neurologist in order to assess the practical significance of this MRI finding.
