What Does Short Femur And Humerus Around 35th Week Of Pregnancy Indicate?

Question: I have a question about my wifes pregnancy. She is 35 weeks pregnant. Everything was fine in the pregnancy until her 32 week ultrasound. She had her quad screen at 17 or 18 weeks and all came back negative. She had her 20 week ultrasound and everything was perfectly fine and all limbs, organs and everything looked great BUT her placenta was low so they wanted her to come back for follow up ultrasounds. On her 32 week ultrasound the doctor told her that the femur was measuring the 5th percentile and then on her 35 week ultrasound they doc told her that the femur and humerous was measuring in the 5th percentile.



Could this be dwarfisim or something like that or are there other signs of this that they would have mentioned. The report said everything else is normal.

1) Short femur and humerus around 35th week of pregnancy may indicate constitutional short stature (due to genetic cause). However, we need to rule out asymmetric IUGR (Intrauterine Growth Restriction) in this context. Here, I suggest to repeat ultrasound scan after 1 week and consult with Obstetrician to confirm the diagnosis.

2) As her ultrasound scan was NORMAL up to 32 weeks, then chance of Fetal Dwarfism is Quite LOW at this stage. Constitutional Short Stature or IUGR is the main cause. Skeletal Dysplasia of long bones should be ruled out.

1) YES, short limbs and feature of Dwarfism should be visible in the earlier scan report. It is quite difficult to interpret above findings on 35th weeks pregnancy.

Constitutional Short Stature or Asymmetric IUGR is MORE prevalent in this context.

2) Skeletal dysplasia means disproportionate short stature. If short stature is proportional, the condition may be due to endocrine or metabolic disorders or chromosomal or non-skeletal dysplasia, genetic defects etc.

It is characterized by abnormalities of cartilage and bone growth, resulting in abnormal shape and size of the skeleton and disproportion of the long bones, spine, and head. In general, patients with disproportionately short stature may be present with skeletal dysplasia or Osteochondrodysplasia.
