What Does Presence Of Sugar In Urine When Diagnosed With Colovesical Fistula Suggest?

Question: I have a colovesical fistula. I am awaiting an appointment to see a surgeon. In the meantime I have a few questions
1) When I had a urine test for the UTI (before we figured out I had the fistula that was causing the UTI) they detected sugar in the urine - yet to my knoledge I have not got diabetes. Is it possible the sugar was from material in bowel?

2) I now pee out poop and gas and small bits of veg. Does everything get out of the bladder or will some things just stay there until the operate on me?

3) What is a good pain killer for me to ask for - sometimes this all hurts - and I find advil helps stop frequent urination and pain - but I hear it can cause a GI bleed. Tylonal seems to only do a bit of pain reduction. Any good safe recommendations for a prescription pan killer to take once and a while when things hurt?

4) the CT scan showed I have the fistula -- it didn't any reason why it formed but I am pretty sure I have diverticulosis and I had a mild attack just prior to the fistula forming. Is there a reason that a CT scan would NOT show diverticular pouches etc. on the sigmoid colon when you in fact do have them? I've had attacks in the past that resolved with Flagyl so I am pretty sure I have diverticulosis but was never tested formally for it.

5) What other symptoms would I be experiencing if this were due to an advanced sigmoid carcinoma -- I don't really have many symptoms except for the peeing of pop and gas and food bits and some occasional pain and UTI - so I am trying to rule it out as a minor chance.

6) It will be weeks and maybe months until they operate on me - how long can i take cipro for without danger.

This does not seem to be cancer caused.

Detailed Answer:
Hello!

Thank you for the query.

1. Sugars are digested very quickly and absorbed in the small intestine. So in normal conditions (with healthy pancreas) there is no way sugars can appear in the large intestine. So it is impossible that the sugar was from the intestine.

2.Usually such fistula is just a tiny hole. Through this hole only small parts can travel to the bladder. So most likely you are removing everything with the urine so no solid particles stay in the bladder.

3. If Advil is helpful for you, no need to seek for another medicine. Instead of that, you may add an antiacid medicine which will protect your stomach (like Omperasole). If you still do not want to try it in such configuration, Vicodin is the next medicine which wont cause stomach problems.

4.Actually CT is not the best method for large intestine visualization. To have a good image of the large intestine in the CT the intestine must be cleared and the 2 hours of contrast drinking is necessary before the CT. Additionally, the gas is delivered to the large intestine right before such CT. Without all this factors, diverticles can be simply invisible.

5. As sigmoid intestine contains usually solid stool, its cancer gives an obstruction as a common symptom. Other very common symptom of any colon cancer is low hemoglobin. And it is rare for colon cancer to cause fistula.

6. There is no any specific time for ciproloxacine intake. If its effective, should be taken as long as necessary. It protects yours kidneys.
