Question: Hello Doctors. I had a high risk encounter with a sex worker in Panama bout 3 month ago. im not sure what we did I was very drunk and don't remember exactly what we did if anything i might have gone down on her but I'm not sure. I got an STD panel done that had HEP B and C HIV Antibody blood test RPR for Syphilis HSV1 HSV 2 Chlamydiae and Gonorrhea all were non detectied or non reactive accept the hep C which had a 0.02 titter. I got tested again at two months for HEP B and C which had a titter of 0.01 Had a 4th gen. antigen antibody 1/2 HIV test non reactive and RPR for Syphilis with confirmatory non reactive . Over this time period i have been very stressed iv lost almost 30 pounds in the last 3 months, I developed trush at about 4 weeks after the encounter and around the same time had a small dry dark mole on my penis which has faded, and in the past couple of days 2 pin head dark spots on the head of my penis. i haven't noticed any cancer sore on my penis or in or around my mouth. I haven't felt sick accept for two days after the encounter on the boat i work on it was rough seas and i had nite sweats that nite for a couple of hours and got pain in the left side of my pelvis that lasted a few days, I get motion sickness bad. I haven't had and fevers or diarrhea or vomiting or Nausea. I have psoriasis and it cleared up on my hand about a month after the encounter . 1 can I trust my test results at 8 weeks? 2, can all the symptoms I've had be from stress? My Stress level has been to the point of Suicidal. Any thing will help. Thanks so much.

Brief Answer:
Syphilis, Gonorrhea and Chlamydia are unlikely.

Detailed Answer:

Thanks for posting your query on HCM.
I really appreciate your apprehension and anxiety over the things happening for the last couple of weeks.

The treponemal test for syphilis RPR is highly sensitive and thus the possibility of Syphilis is ruled out.

The lab tests for syphilis, gonorrhea and chlamydia can be conclusive at this time period hence they are ruled out.

Regarding blood tests for HIV & Hepatitis C, these tests can not detect HIV/HCV in the blood soon after infection because it takes time for your body to make these antibodies. In some cases it can take up to six months.
So, the tests for HIV/HCV would be more conclusive by 12 weeks.
Your present symptoms have no relation to HIV.

The dark spots on penis should be examined properly to confirm or rule out HPV infection for that you need a consultation with a dermatologist or venereologist.

Now a days most of the escorts or comforters or commercial sex workers are educated and health conscious, from time to time they get the necessary instructions and lab tests done to detect an early infection and get treated.
The chances of contracting the infection with a single encounter under the influence of alcohol on the boat and that too you are not sure of having penetrative sex is minimal.

It is apparent from the problems described in your query, you are quite tensed
about the STDs.

Relax, most of the STDs are effectively treated with the use of available drugs.
Most of your symptoms are suggestive of undue stress, so try to avoid stress and take an opinion of a psychiatrist for the suicidal thoughts which need to be controlled and treated.

Avoid alcohol because chronic alcoholism has its own adverse impact.
Use single or double condoms in future to protect yourself from the STDs & unwanted pregnancy for your partner.

To be more conclusive repeat the tests for HIV,HEPATITIS B ad C at 12 weeks after your risky sexual encounter.

Hope the suggestions given above could be useful not only getting relief from the undue stress and anxiety but also valuable advice for your future.

Best Regards.
