What Does My Serum IgE Levels Investigation Result Report Indicate?

Question: Hello

I am 61 year old female. I have been going through allergy treatment for last 7 months. In January this year I was having sporadic itchy rashes, which were not too serious. the doctor conducted Serum IgE investigation. The result was 800.95 IU/ml. My doctor told me to avoid a list of possible allergens and gave me one Teczine 10 mg one at night for a month. He also prescribed me a lotion with following elements:

Menthol 1%
Camphor 1%
Clop M cream - 100 G
Uthbless cream - 40 G
Crotarax HC cream - 100 G
Betnovate GM cream - 100 G

I followed the prescription. After a month, I went to him again. This time, he prescribed me a new mixture:

Betnovate GM cream - 100G
Nevlon lotion - 200ML
Clonate lotion - 100ML

He also gave me allegra 120 1 at day and 1 at night for 10 days after eating. And Ascert D 1 at day and 1 at night for 10 days after eating.

I was on these medicines till March this year when he gave me Suncros soft gel 50G for using twice a day.

In XXXXXXX he brought a little change in the medication. He suggested me another mixture with following elements:

Menthol 1%
Camphor 1%
Betnovate GM cream 100G
Olesoft lotion 200G
Clonate lotion 100ML

He told me to rub it over my body two times a day for 2 months. He also gave me new medicines:
Allegra M
ATARAX-25 Tab
Pan D

In July 2016, he conducted another Serum IgE investigation. The result was 582 IU/ML.

However my rashes have worsen. It started with my palms. In the middle of July, my palms have become itchy and painful. Now I have developed severely itchy rashes all over my body. The strange thing is, when I am itching a part of my body, that part is quickly turning into fluid-filled blister. The blisters are painful. My palms were full of scabs a week or two ago. The doctor gave me the following cream to use on my palms:

Menthol 1%
Camphor 1%
Betnovate GM cream 100G
Lijlibet cream 100G
Technovate M cream 60G
After using the cream, the itching has lessened a bit and the discoloration has gone. But the itching is back. My palms are very painful. The pain is severe, it feels like the pain is coming from the bones. My whole body is itchy and new small fluid-filled blisters are popping up all of a sudden whenever I itch a body part. The blisters crack and the fluid comes out, then after a few days, it becomes dry.

My doctor is clueless what it is. Does allergy cause fluid-filled blisters both large and small? Can it be something else? I had chicken pox a year ago and was on Zovirax. I am attaching few images here so that you get a better understanding of my condition.

Brief Answer:
Bullous impetigo

Detailed Answer:
Hi,

Thanks for your query.

I have seen the images you have attached.

The lesions appear to be due to Bullous Impetigo.

Bullous Impetigo occurs due to bacteria called Staphylococcus aureus. This can occur over a underlying allergic condition. In allergic conditions, constant scratching can inoculate the bacteria into the skin and lead to Bullous impetigo.

I would recommend you to get a culture and antibiotic sensitivity test of the liquid ooze from the broken lesions. This test grows the bacteria present in the lesions, identify the strain and identify the antibiotic to which it is susceptible.

It is not advisable to use steroid creams like Betnovate or Technovate now as they reduce the local skin immunity and make skin more prone to bacterial infections.

Hope, I answered your query.

Wish you good health.

Hello

Bullous Impetigo affects children most. Can it affect someone who is 60+ year old?

Also, I have learned that the disease is contagious. But other people in my family haven't been affected by it. How is that possible?

Finally, does it heal on its own?

Kindly answer these questions.

Brief Answer:
Does not heal on own

Detailed Answer:
Hi,

Thanks for writing back.

Bullous impetigo is common in newborns but not exclusively found in them. Adult and old age too are affected.

Yes, it is contagious as it harbors bacteria causing infection. The spread is proportional to its severity. With the severity you have, I do not believe it can affect others at this time.

No, does not heal on its won. Specific antibiotics are needed. Hence, I suggested culture and antibiotic sensitivity test to identify the bacteria, confirm the diagnosis and take suitable antibiotics as suggested by the test.

Hope, I answered your query.

Wish you good health.

Hello

I went to a clinic for a culture and antibiotic sensitivity test. They wanted to know the specimen or the sample. So please let me know everything related to the tests in medical terms so I can properly convey everything to the pathology department of the clinic.

The lesions keep popping up and I feel an intense burning sensation in my palm and under my feet. Can you suggest any anti-inflammatory medicine?

Also, let me know the complications the disease might cause to elderly people. Is outpatient treatment enough or is hospitalization required?

Kindly answer.

Sorry for the delay in my response.

Sample is the liquid oozing after the lesion is broken. Ask for a swab culture and sensitivity test of the fluid from the lesions.

Yes, you can take Tab Motrin 400mg twice daily after food for three days.

Since, its infective older or younger are at risk of getting infection. More chances in people having skin problems previously.

Out patient treatment is all it takes.

Hello

I have done the liquid oozing test from a hospital and got the report. The report only shows the following:

Fibrocollagenous tissue infiltrated and lyenthomonoun clear cells

What does it mean?

I have been on steroid and amoxicillin. The pain and itching have reduced. Small blisters are still popping up but the duration of their lifetime is now less than before.

Brief Answer:
Yes, for steroids

Detailed Answer:
Hi,

Tanks for writing back.

Based on the reports, your condition can be narrowed down to a autoimmune cause.

Yes, long term steroids is the only best choice to suppress the immune system and reduce the lesions.

Need not worry, steroids would be of low dose.

Wish you good health.
