What Does My Lab Test Result Indicate?

Question: The dog bite my child on 15/02/2016, rabipur inj given on 15 Feb, 18 Feb, 22 Feb, 29 Feb & 14 Mar. I also administer Berirab P inj on 23 Feb, although it is late. Rabies virus antibodies antibody serum test done by me. the blood sample took on 01/04/2016, report generated on 26/04/2016. The report observed value is 0.45. What should I do now? Please help.

Hello,
Thanks for posting in HCM.
I saw the report and also understood your query.
From the info its clear that its almost now more than 2 months that dog had bitten the kid.
Also from the dates you mentioned it is evident that anti rabies injections were taken starting the same day of the bite.
Usually for immunity to reflect in the report will take time as the age of kid is only 4.5 yrs.
I would like to know how is the kid? Does he have any neurological problem?
Is he eating well and active enough?
Kindly answer me these questions.

My son is fine and active enough. he is eating normally and have no neurological problem till date. Dr AK XXXXXXX Rabies specialist (Delhi) advice me to repeat the rabies vaccination course. What is your opinion?

Thanks for following up.
Usually after taking the vaccines and completing the course, there is actually no need to this test. But as you have already done it and test result is very near to borderline immune state of the kid to rabies.
I personally feel there is no need to repeat the whole course of the vaccination. If child is normal there is no need to fear for rabies.
I forgot to ask about -1. Whether the dog is established to be rabid or died after biting?
2. What was the nature of dog bite and where it bit on kid's body? for eg. was it just scratch without bleed? or bite with bleeding?
Kindly answer me these last 2 questions. Sorry for asking again but it will be useful.

The case has also been discussed with Dr Diptanshu XXXXXXX one of your expert. Although I want to share the same with you also.

In the month of Feb, my 4y 4m only son bitten by a stray dog (female) on his hips around 1.30 pm. He is wearing two pants. He got two wounds. Small on upper part and severe on bottom part with blood spot on pant. Cleaning not done properly, only savlon used. Took him to govt hospital emergency, Dr categories it cat-II, but it was cat-III. RABIPUR given on 0, 3, 7 day. 7th day I came to know that the dog died two days before, people confirm about his madness also. 8th day, I went to a private clinic, Dr also confirm it cat 3 case and advice to have Berirab P inj around his wound, though it is late and complete Rebipur doses for 14th and 28th day. Rabipur course is over.


is there any side effect, if I go for repeat the rabipur vaccination?

Thanks for following up.
Thanks again to tell me the whole scenario from start. The information you provided was important.
Seeing this new information you gave me I also will agree with the doctor to repeat the vaccines again just to be on safer side.
As such rabies vaccines are 100% effective when taken promptly after exposure which you have done. Also you have taken the anti rabies immunoglobulin which was a very good thing to do.
As the deciding value is 0.5 of the report and test value has come as 0.45 it is on very borderline to call it positive or negative.
As a doctor I would still say that additional vaccines are not required and you need not worry about getting rabies infection to your son.
But getting another round of vaccination will not be harmful either. There are very minimal side effects like pain at inj site and some body ache and nausea. Nothing of serious note.
From the history of the bite and possibility of dog being rabid, it will be better you do another round of vaccination. And do not worry after finishing the course to do test for RRFIT. Just finish the doses and leave it. Because the values depends on lots of factors to come right.
Your kid is 99.99% protected even right now. But we do not want to take that 0.01% chance either. So go ahead and take another round of vaccines.
