What Does My Lab Test Report Indicate?

Question: I'm writing this post as I need an opinion / help on what to do next as I'm feeling lost. Important notes Symptoms have been ongoing for over 6 months now, and I don't have health insurance so have been a cash patient with all of the below.

It all started from what I recall as a result of a one night stand with a female (unprotected)- roughly 1 week later I had a burning sensation in testicles / swelling. I went to get an STD test at a local clinic and at the same time got a shot of Cipro as well as Z-pak (erythromycin 5 days). Blood was drawn and urinalysis was performed. STD tests all came back negative, swelling went away temporarily in testicles, however the following week lower back pain came in to the point where I couldn't sleep (for the record I've never had lower back pain before with the exception of sports related activity, this was unlike anything i've ever experienced) and I began to notice what seemed like mostly either green/black particles (sometimes other colors as well) / chunks in my semen, or coiled, sometimes straightened hair like particles in my semen. I've read everything I could on-line on the matter - it wasn't residue from clothes or dirty hands, it happened in the shower (with clean hands) and anywhere else i'd ejaculate every single time.

Seeing as all STD tests were negative, I went to a top rated urologist in the area who conducted a prostate exam, did a prostate culture and another urinary analysis (I showed him pictures of the discharge and described my symptoms at the time).

Test came back negative, at this point back pain was mostly persistent (some points in the day were better than others) and now I would get pain in the testicles when I ejaculated.

This now being roughly 2 months in from when symptoms began I went to get another STD test (urinary test and blood drawn). Again everything came back negative.

I then did research on my own online and asked my general practitioner to write me a script for a local lab to do a semen analysis (Ureaplasma/Mycoplasma hominis & Body Fluid Culture, Sterile). Both came back negative

I then asked my doctor to write me a script for a testicular sonogram. The results of which were bilateral cysts in testicles and varicocele veins on one side.

I then went to an infectious disease specialist, showed him my list of results, symptoms, pictures etc. He again ordered a semen culture to be done (one being listed above, and another new one - anaerobic and aerobic bacteria) both came back negative.

I'm not sure what to do next, or what urologist / doctor to see next. I've left out points about interactions with urologists and labs being beyond difficult, the fact that you can't even begin to explain what your symptoms are / to see what the course of action is next without first shelling out $350+ on a visit only to see someone for a few minutes who has no answer for you besides the same generic one you've heard before, and to order the same test(s) you've already done. I'm out of pocket what for me is a pretty substantial amount, and just not sure what do moving forward.

Back pain has subsided, what I at first thought was possibly a gym related injury clearly was not. I still sometimes have pain when I ejaculate in testicle(s), and there is the discharge pictured every single time I ejaculate.

Can someone advise on what type of specialist I should seek out next? or what test to perform that i'm unaware of?

I have gone carefully through your query and understand your concerns. You have gone to a number of specialists and the tests have all pointed to the fact that you do not exhibit signs of STDs. The main problem is that you are freaking out. Repeatedly seeking assistance from various types of doctors would not change the scenario. Rater, you should accept the reports to be correct and should relax. It is not essential that any complaint that you have to point to STDs. But it is definitely possible that your anxiety could give rise to new symptoms (ones that you actually develop and exhibit).

The specialist that you should seek is a psychiatrist as well as a psychologist. This does not mean that you have any mental problem. This just means that the mental bias and the anxiety are complicating the presentations and therefore need to be relieved if you are to get to the root of all problems. It is only after the anxiety relief that it can be decided if you need further testing and further specialist consultation. In fact, you would then be able to decide on it yourself.
