Under distended means, there is not enough filling up...

And, in stomach under distension means there is not much of gas available. Because gas is the one which causes distension in the stomach.

In the bladder, considering its a urinary bladder, under distended means, there is not much of urine present when the test is being performed like CT scan, Ultrasound scan etc...
