What Does My Blood Test Report Indicate?

Question: Hi Doc ,
My wife is pregnant for 6 weeks and when done complete blood test the report shows
HIV - Negative
Hbsag - weakly reactive

What does it mean if it's weakly reactive?? Can this be made non reactive?
Will it affect my kid? If vaccine is taken now is it curable??
The lab guy asked me to do another test elcia test I guess m not sure..

Current test is equivocal, please go for ELISA testing.

Thank you for uploading the relevant report.
I have reviewed your report in detail.
The current report has tested for Hepatitis B in your wife using the Rapid SPOT test.
This utilizes a technique called Immuno Chromatography.
It is not as reliable as the ELISA test.
The ELISA test is much more reliable and conclusive for testing against Hepatitis B.
Based on the current report, do not get worried and do not panic.
It is better to get the ELISA test done and then revert to me.
I do not want to alarm you or scare you regarding Hepatitis B in pregnancy based on an inconclusive and unreliable test.
Please proceed with the ELISA test and revert to me once you get that report in hand.
Please do not worry right now as this test often gives an erroneous result.

Thanks for your reply since hiv test also was done with the same technique and it showed negative can I still rely on that or even for hiv I need ELISA test??

No need to repeat HIV test since it is conclusively negative.

When the test is negative , it is reliably negative.
WHen it is weakly positive, that is , when it is more likely to be false.
SO there is no need to repeat the HIV test, since it is conclusively negative.

Hi XXXXXXX hope this is a follow up Session on my Wife's Report.
we have done the ELISA test for HBsAg (reports Uploaded) and the results Came as Negative
However the Doc here Suggested to do even HBe Ag and HBc Ag test to check if the infections could ne transmitted to the baby
I have Uploaded the report/pic of HBe Ag and HBc Ag where HBe Ag shows Negative and HBc Ag shows positive (this is by seeing the reference Range)
Could you please Check the reports and reply back?
Also LFT was done and ive uploaded that too.
M really worried Since HBc Ag is not within the Refrence range and looks like its positive

Hi I've uploaded LFT report please review !!!
01) jus to add few things, the Doc asked me to test my HBsAg and it came back negative
02) My wife is working as a staff nurse .
And once she joined the hospital as staff nurse they aske her/everyone to take Hepatisis B vaccine which she did not take

03) sine HBcAg (Hepatisis B core antigen)
Showing as positive , is it something to worry ? Can it be cured? Can it be prevented from getting transmitted to the baby??

Hello again.
I have gone through all the reports.
Now here is the explanation.
Hepatitis B has 3 antigens - surface, core and E antigen.
Only core antigen is positive in her case.
The possibilities with this picture are -
1. Resolved infection ( immune to Hepatitis B )
2. Very early infection or chronic infection.
3. False positive Hb core antigen antibody.
NOw to differentiate, further tests are needed -
1. Repeat of Hb core antigen antibody at another lab.
2. HBV DNA estimation
If HBV DNA is negative, and the core antibody is still present, then she is immune.
However, further vaccination should be given to her irrespective of the tests, since she is a staff nurse and not vaccinated ( she should have taken the vaccine before joining the hospital ).
As such, she is not infective to the baby, as the surface antigen is negative, but still, the vaccine prevents infection in 90- 100 % of cases.
There is nothing to worry.
You must have protected sex till further testing is done, and a clear consensus is reached.
You can revert to me with the tests suggested.
Also, please consult a leading Gastro enterologist in your city, and get his / her opinion on this also, as this is beyond the domain of most obstetricians.

Thanks for your reply.. How about the LFT?? are they normal?? Does vaccinating the baby within 12 hrs of delivery help the baby not to get this infection invade if my wife is infectious ??
And for HBsAg is ELISA method is reliable 100%??

Yes, the LFT is quite normal.
Vaccinating the baby would further protect the child, but if the workup shows that your wife is not infectious, and she gets vaccinated, there is no need to vaccinate the child immediately after birth.
Yes, HBsAg ELISA is 100 % reliable.
