Hello Dr.Sharka,

This is XXXXXXX back again. Seasons' Greetings and Happy New Year to you.
Things are improving with me. I have been off work since Oct.14,2016. Stress level
has subsided considerably.
Roemheld syndrome is under control with the eating adjustments made with
your recommendations.
Still haven't seen my cardiologist for initial appointment not till March 15,2017.
Appears that extremely high stress; over the past year might have played
a big factor on my health conditions.
Another health issue I didn't previously mention to you was a near collapse at
work due to my legs giving up while I was just standing.
I have been doing physiotherapy for the past two months working on the right
gluteus medius which appears to have been the main cause initially. Also general
strengthening lower back.quads,lats,etc.
I did private MRI on XXXXXXX 06,2017 for L-Spine and Sacrum; and, SI Joints.
I now have the MRI report and a CD.

Finally, here is my question to you. Do you also do radiology consultation
and MRI interpretation?
If not, can you please recommend a highly rated Radiologist with HCM?

Hello Again Dr.Iliri,

Have set-up the two page MRI analysis from mic. I have spent a fair
amount of time googling the meaning of all the medical words and
some very good blogs on spinal degeneration,etc.
Probably of most concern is 1. Marrow signal is mildly heterogenous.
2. There is incidental hemangioma in the L1 vertebral body.
3. There are tiny Schmorl's nodes along the vertebral endplates at L4-5.

My plan is to continue to work for the college. I have been doing a
custodial job at the college after my retirement from a corporate career in
sales and management. Would also appreciate your opinion on this.

I understand your concern and would like to reassure you that these MRI findings are not indicative of any concerning disorder.

Let me explain as follows:

1. Marrow signal is mildly heterogenous: This means that there are some chronic changes of the bones of the vertebral column, probably related to aging and osteoporosis. These findings are quite normal for your age. You should know that they occur in every person and are asymptomatic.

2. There is incidental hemangioma in the L1 vertebral body: This indicates a benign tumor (which may also be not considered a tumor, but a born defect or a misplacement of some blood vessels during fetal development), formed by circular blood vessels inside the body of the vertebral bone. You have been born with it. It is similar to the red spots in the skin in different persons, which are visible. But, it does not indicate anything abnormal. It does not lead to any clinical symptoms and it has no progression. That is why it is called incidental finding.

3-There are tiny Schmorl's nodes along the vertebral endplates at L4-5 : This indicates chronic degeneration of the intervertebral disc, which has a soft liquid part and a hard fibrous part. This Shcmorl's nodes are related to the protrusion of the soft liquid part of the disk into the adjacent vertebral body. They do not cause any radicular pain or nerve compression. So, it is just part of the chronic degeneration of the vertebral column related to aging and wrong posture.

From the other hand, I would like to explain some other findings in your MRI:
3. Mild foraminal narrowing at L3-4 and L5-S1. Moderate left and mild right foraminal nenowing at L'1-5.

These findings indicate mild and moderate intervertebral disc bulging in three disc levels, which can cause nerve compression. They are not concerning right now, but can further progress into herniation and radicular pain (irradiating in the inferior limbs).

That is why, it is necessary performing a lot of physiotherapy to reinforce the back muscles and avoid prolonged sitting or standing position. It is important correcting your posture and avoid weight lifting. All these tips will help avoid further progression.

Anyway, the most important thing in this report is the there is no compression of the spinal cord (no central stenosis) and there is no discal herniation.

There is no need consulting with a neurosurgeon, as all these findings can be maintained under control for the moment with the above mentioned tips.
