Question: I am an athletically very active 59 year old male. I have recently experienced sciatica in both legs. I have had no physical trauma to my back. The attached image is from a lumbar MRI performed on my back this month. What is your assessment of the appearance of the L4-L5 disc and the L5 vertibrae, and what is your recommendation for relieving the sciatica, which appears to be related to this?

There is spondylolysthesis.

Detailed Answer:
Hi, I had gone through your question and understand your concerns.

From MRI images that you uploaded, there is spondylolysthesis ( slipped vertebrae) at L4/L5 level with moderate stenosis ( narrowing) of spinal canal at this level.

L5 vertebra is slipped toward regaining to L4.

In my opinion there's need for x-Ray films of your lumbar column in neutral, extending and flexing posture to evaluate correctly grade of spondylolysthesis.

Treatment is mainly surgical ( spinal fusion) and is guided by your symptoms in correlation with imaging findings.
