Question: Need someone to explain in details what all this means in a simple language that I can read and make sense to me. I have provided an MRI SCAN

I have read through your query in detail.
Please find the MRI scan brain explained in simple language below.

There is a 5.3 x 5.0 x 4.5 cm spherical mass (space occupying lesion) in the right half of the brain. It is showing characteristics of central dead tissue surrounded by peripheral areas of tissue showing active blood flow. It is causing increase in pressure inside the head and this is seen as the left brain being compressed by the spherical mass accompanied with swelling (oedema) of surrounding brain. A midline shift of 0.4 mm is the upper limit of normal and in this scan we find the brain is shifted 1.29 cm off the midline to the left side, and is critical for the patient.

It is also mentioned that the mass is infiltrating into the corpus callosum, as area integrating the right and left part of brain and very important for normal brain function.

With this information, the first possibility is a high grade malignancy of brain (GBM) and this is a very aggressive cancerous tumor.

The other possibilities mentioned are an infection abscess or a white blood cell related cancer involving the brain (Primary CNS lymphoma).

In the given circumstances, the patient needs emergency care and decompression of the increased brain pressure to save his life. The findings also have to be analysed in context to the symptoms of the patient course of illness. If it is indeed a cancer then the treatment is difficult.

what methods can be used to decompress the brain? when young the patient had a scar in the brain which made him fit a lot, but medication was used Tegretol to help with the fit. it helped and the patient never fitted for a long time.

it was only recently he fell ill and had a XXXXXXX stoke which afected the left side of his body, ( he his left handed).
I am also told that he has high cholestrol level as well. please can you take a look at the other results and tell me if there is something abnormal about it

The methods to decompress are by giving medicines like mannitol and steroids to reduce the brain swelling and also doing surgery if the patient condition is stable.

The scar in brain which the person had in childhood must be explored in detail. Tegretol is an anti seizure medication.

The cholesterol levels are mildly high and should not be a limiting factor in treating his condition. The liver enzyme is also slightly elevated and not a concern.

I hope his doctors are well informed of his earlier history of fits and the brain scar which you have mentioned.

One question in my mind is if he has been having fever and neck rigidity in the recent days, this can indicate an infective condition.

Also his immune status needs to be verified, if he is having a decreased immunity.

thanks for getting back to me, yes he had a fever at the time. I was told he felt drowsy almost slumming on the phone, sweating. Was rushed to the hospital and was treated for thypoid fever and malaria.

When in hospital they discovred he could not move his left arm and left leg ( he his left handed) , so termed it a XXXXXXX stroke.

We were advised to have this MRI scan to investigate further which we did. Is this an indication of an infective condition?

We have a slight dilemma here. Since he had fever, and infection is a possibility but then the infiltration into corpus callosum leads us into thinking of a malignancy. His condition might need to be reassessed by imaging again after some days. Probably the doctors will also give antibiotic medicines and see the response.

However, whether it is malignancy or infection, his condition is critical at the moment and something surely needs to be done about the brain swelling.
