In my opinion treatment for medular syringo-hydromyelia should be guided by symptoms it causes.
If you feel difficulty walking, urination problems, leg numbness etc. there is need for treatment.
Treatment is surgical and consists on shunting hydromyelia cavity.
Another thing to evaluate is any associated Chiari malformation and this needs craniocervical MRI.
If you feel no or only mild symptoms, then imaging follow up with MRI is sufficient.
I think first follow up MRI should be done after six months.
