Hi,

Most commonly increased echogenicity means there is increased fat in the liver.In other words fatty liver changes are the most common cause of increased liver echogenicity in a asymptomatic patient.
Poor penetration means that resolution of the images are not very good.Most common cause of poor resolution is thickened abdominal wall.
