Posted on Tue, 28 Apr 2015
Question: hi doc, my 6.5 months old son had a high temperature last night. He was looking tired from morning. but all his activities, feeding and motion is normal. today morning also he is looking tired sometimes. he doesn't have cold or cough and temperature is also normal. i am checking his temperature every 2 hrs and it is between 35.5 and 36.8 degree celcius. we are staying at XXXXXXX and our flat is in top floor. the climate is too hot for the last two days and it is not bearable for adults also.24/7 we are using fans. i guess the fever is due to climate change. what is ur opinion doc? what may be the reason? and what are the things i need to take care?
i am giving ragi porridge and 1 orange daily to my baby apart from breast milk. is it fine?
also my baby birth weight was 3 kg. in february his weight was 6 kg and yesterday when i checked it is 5.5 kg.
day before yesterday we gave polio drops to my baby. it will cause fever?

Brief Answer:
It appears to be due to warm climate

Detailed Answer:
Hello XXXXXXX

Thanks for the query

First of all I would like to tell you that you are a very good observer and also thanks for giving such a nice history.
You are right.According to the situation you have mentioned with loss of weight and his good activity, all this goes in favor of environmental/dehydration fever.
The thermoregulatory mechanism of an infant differs from that of an adult and it is also not well developed. So infants are more prone to develop fever following exposure to warm environment. The temperature of 35.5 to 36.8 is normal for an infant.
I would suggest you to change your house if it is possible.
The comfortable temperature for an infant is around 26-29 C.Use of an air conditioner will help.
In the case you are using a fan use a mosquito net and then put a wet thin cotton cloth(like dhoti) over it.This works better than air cooler and I advise this to all my patients.
Keep keep him hydrated well with plenty of fluids.Avoid overdressing.Use thin cotton cloths only.Observe for urinary frequency. The antipyratic does not help in this condition.
Also observe for any other associated symptoms like cough/cold or loose stool as the possibility of any viral infection can not be rule out at this stage.
Unlike other injectable vaccine polio vaccine usually does not causes fever.
The best option is to change the house if you are in a rented one,as the peak summer has not arrived yet.
I would be glad to answer if you have any further query.

Detailed Answer:
Hi

Don't worry. Its good that you have detected it at early stage.Give him frequent sips of fluids(water/milk/ORS).
If oral acceptance is good he will recover.
Avoid precipitating factor, take the measures to prevent further exposure from hot environment.

Brief Answer:
I agree with him

Detailed Answer:
Hello

The time when I was answering your query there was no cough and cold.
Fever may be the only initial symptom of any viral infection.Other symptoms like cough/cold or loose stool may develop within 2-3 days.
The same happened to your baby.
I agree with Dr XXXXXXX He has given a very precise and right advice.
Keep an eye over his symptoms and if needed get your baby evaluated by a pediatrician.

thanks doc.
today we went to paediatrician and he told its viral infection.
he prescribed sinarest 0.4 ml(1-1-1). is it safe for the baby?
medicine is compulsory to treat viral infection or it will go off naturally?
anyway i gave him 0.4 ml now.
doctor
Answered by Dr. Pradeep Kumar Jain (13 minutes later)
Brief Answer:
You can give sinarest for 2-3 days

Detailed Answer:
Most of the viral infections do not need any specific treatment(as antibiotic in bacterial infection to kill the bacteria).Treatment should be given for the associated symptoms like cough/cold for symptomatic relief and to avoid disturbance in daily activity and feeding.
Sinarest contains antiallergic and decongestant. As your baby is suffering from cold and sneezing you can give the medication in the prescribed dosage for two to three days.
