default
Posted on Thu, 26 Jun 2014
Question: I just went to the doctor because i had two brown scabs on my penis. I thought it might be dirt so i scraped off the scab and now it looks like a lesion. The er doctor and care now doctor both didn't think it was herpes but the care now doctor tested blood, urine, and for herpes. The care now doctor thinks its a yeast infection, while the er doctor just thought it was a cut.

Since when do you have these?
Any outside contact in the recent past?
Any similar lesions in the past?

Brief Answer:
Looks like traumatic ulcer, local care required

Detailed Answer:
Hi again

I have seen the attached pictures.
There are two small ulcers present over the penile shaft.
Now most importantly, it is surely not looking like a fungal infection.
The pictures are not very clear but the possibilities that come to my mind are that either it is a trauma induced ulceration or it could be herpes.
My first possibility is surely that of a trauma induced infection.
Now to rule out herpes, I would like to know if you noticed development of any fluid filled blisters over the affected areas prior to the scabbing that you noticed.
Also since you have already been tested for herpes,I am sure the blood reports will be of help to us in confirmation of the diagnosis.
At present, local cleaning of the affected area is very important.
You need to clean the area with lukewarm saline water and apply a topical antibiotic cream like neosporin or mupirocin over the affected areas twice a day.
This will help in controlling the local infection.
I am sure it should heal with proper local antiseptic care.

No, thats the thing, i didn't even notice the two ulcers until after the scabbed up. So there was no fluid or anything present. I am using Nystatin cream. Will that help?

Brief Answer:
Need treatment with topical antibiotic

Detailed Answer:
Hi again

So I would like to keep the possibility of a traumatic ulcer first owing to on history we development of any blisters.
Nystatin might not really help you much since it is an antifungal agent and I do not see signs of any fungal infection.
You can rather start with application of medicine as advised.
In case the lesion tends to persist or increase even after using the medicines for three to five days, then you might need further evaluation to rule out sexually transmitted diseases like herpes or chancroid, although I feel the possibility of these is very less .
