Question: My husband has just discovered a lump above his right nipple. It is not a hard nodule. It is more like a swelled up lump. He is 66 years old and in good health otherwise.

To recapitulate: Male/66 - just discovered a lump above right nipple - not a hard nodule - more like a swelled up lump ...
The possible causes as per the history are:
Retention cyst like a sebaceous cyst.
Possibility of a cancer should always be remembered unless proved otherwise.

This can be done by:
Clinical evaluation, examination by a Doctor, preferably by a General Surgeon.
FNAC and/or excision biopsy is confirmatory of the diagnosis and a treatment in itself if this is a cyst.
I would advise you to go for it for 2 reasons:
1. One gets rid of the lump that is not a normal part for the body.
2. One gets a definitive diagnosis.
