I have gone through your query and I have also seen the photograph
In the arm pits I can see dome shaped, skin colored to reddish bumps. I will keep a possibility of Fox-Fordyce Disease. It is a very itchy condition.
I would like to know more from you regarding lesions affecting your armpit.
-Does itching involve both the arm pits?
-Have you noticed that you have reduced sweating from the arm pits?
-Have you tried any medication?

In this photograph I can see a raised red margin at the upper left corner of the photograph, some scaling and central clearing; this is most likely a fungal infection.
However, I would be more sure about my diagnosis if you can upload another one.
-Also let me know where exactly it is situated on the legs?

Treat for scabies; oral+topical antifungal

Hi.
I will like to treat you empirically for scabies.
Scabies usually presents as itchy bumps at various body sites like arms, legs, genitalis, abdomen and trunk.
The itching is specially troublesome at night.
In my patients of scabies I usually treat them a single whole body application of 5% Permethrin lotion, kept overnight for 8 hours, followed by scrub bath next day.
Alternatively a single oral dose of Ivermectol is also an option.
Other family members if affected by itching should also be treated simultaneously.
You still seem to have Fungal Infection of the groin folds (Tinea Cruris).
If I was the treating doctor I would have started you on oral antifungals e.g either fluconazole or terbinafine in addition to the topical cream that you have been already using.
Oral antifungals are faster acting and control the infection much better when used in combination with topical antifungal creams. Topical creams alone are sometimes not totally effective if the area of involvement is extensive.
Both oral and topical treatment must be continued for 4-6 weeks.

Antifungal tablets are usually recommended for 4-6 weeks. There are no side effects.
Thereafter you may continue with an antifungal dusting powder for maintenance.
Either Permethrin Or Ivermectol is to be used; not both. They don't have any side effects.
