Question: Dear Doctors,

Writing in order to get a second opinion on a recent situation regarding potential HIV exposure. The exposure occurred on 25th of October 2015 and I was the receptive partner. Condom was used and after the ejaculation seemed intact. My worries appeared since about 3 weeks after the contact, I had some kind of mild infection (37.1 - 37.5C temperature, sore throat, razor burn like rash on the neck, nasal congestions and according to a local doctor oral thrush case, for which I was given Nystatin). Further my sexual partner behaviour has changed and currently all contact has been interrupted. On the brighter side he did state that he had a quick HIV test and it was negative before we engaged in the act (I have not seen the test results). Now, if the condom was indeed intact, the risk in my case is rather low to non-existent and the symptoms I have were some other mild viral infection. But to be on the safer side I am assuming that there was some condom failure, which was missed (due to the appearance of the above symptoms).
As such I have undergone a few blood tests (laboratory taken serum blood, not finger prick) for various STDs, including HIV. The HIV tests were all 4th generation (according to information given by the two laboratories, where those were done) and results as follows:
29 days SD Bioline (antigen + anti-Hiv 1/2 antibodies) - negative
43 days HIV Combi by Roche (ECLIA ag/ab) - negative
50 days HIV Combi by Roche (ECLIA ag/ab) - negative
57 days SD Bioline (antigen + anti-Hiv 1/2 antibodies) - negative
Also on 57 days negative Hep B, C, Syphilis.
Local guidelines have been approved in 2004 and state that only a 6 months test is considered conclusive. Based on the guidelines from WHO and EU guidelines (which I could find on the internet, as you can expect) the tests are considered reliable at 4 weeks and only in case of high risk scenario should be repeated at 8 weeks.

As such I would ask for a second opinion regarding the taken tests in light of the high risk exposure (assuming condom failure). According to local guidelines and clinics, the tests would be conclusive at 6 months. Only other medical condition that I have is higher levels of uric acid and possible gout, as well as slightly enlarged liver (fatty liver).

Attached I am sending a full blood works result taken at the beginning of December. I am sorry that it is partially in Bulgarian, but most of the tests have also their respective names written in English. First number on each row is the result and the second one is the referent value of the laboratory. The STD results at the end of the report state "Negative". Further tests have been taken as described above.

Thank you in advance for your answer.

Best regards,
XXXX

Brief Answer:
You are free from Syphilis, Hepatitis B and C, HIV1 infections.

Detailed Answer:
Hi,
Welcome to HCM.
Thanks for posting your query.
Your symptoms are vague, common for everybody and not specific to HIV.
Your test reports reveal that you are free from Syphilis, Hep B and C and HIV 1 infection beyond any doubt. HIV 1 infection is common throughout the world except in certain parts of XXXXXXX up to 99%. HIV 2 infection is prevalent lesser than 1%.
4th generation HIV test is highly reliable and consists of two parts:
1. Antigen part detects HIV 1 infection as early as one week and considered conclusive by 4 weeks.
2. Antibody derection against HIV 1 and 2. This will become conclusive after the window period for HIV (6-8 weeks may be extended up to 12 weeks). If you want to rule out HIV 2 (rarest possibility), then you have repeat the test after 12 weeks time following the risky exposure. As yours is a protected exposure it could not considered as much risky. If the condom had been broken, it would not hold semen when it was withdrawn and can be identified easily, unusual to miss the same. So you need not worry.
Only if the individual are on immunosuppressive drugs or on ART, the antibody formation inside the body (seroconversion) would take more than 12 weeks time. Oterwise the report can be taken as conclusive by 12 weeks time after the exposure.
