What Do These Following BP Readings Indicate?

Question: 73 year old female, 120 lbs. with recent diagnosis of bladder cancer. Taking combination therapy for PRE-diabetes.
Pioglitazone (actos) 15 mg and Januvia 100 mg. Combination calcium channel blocker for hypertension, Azor 5/20 mg. Experiencing hypoglycemia and or hypotension. I feel PCP should review medications. A1c 5.9. BP 86/51 P 70, BP 89/49 P67 bp 85/49 P 61.

I read your query and understood your concerns.

After reviewing your BP levels I should say that it's at the lower limits and your therapy for BP should be reduced.

If you were my patient I would recommend to take Azor 5/20 mg half tablets (half actual dose) and if the BP will persist to stay at actual level I would have stopped it.

Dr. Shehu thank you for your immediate reply. One follow up question. I am concerned about taking Actos due to my bladder cancer diagnosis and feel the combination therapy of Pioglitazone 15 mg and Januvia 100mg is very aggressive for a diagnosis of prediabetes with an A1c result of 5.9%. Actos contradictive with diagnosis of bladder cancer? Experiencing confusion, difficulty concentrating dizziness, lethargic.

Hi back,

Yes they to are a little aggressive for a patient with prediabetes and bladder cancer. If you were my patient I would have stopped Pioglitazone and Januvia immediately.

I would have recommended a healthy lifestyle and diet to control prediabetes and would have restarted only Januvia if the diet isn't effective in controlling blood glucose levels.
