What Do The Following Blood Test And Lipid Profile Results Indicate?
Question: in continuation to my previous query, i would like to mention that ecg was normal echo cardio : valve functioning normal, Jerky IVS motion, Trivial MR. Both ECG and echo were taken after 3 days of commencement of the medicines .
i forgot to mention my height and wt. my weight is 110kgs , height is 5 feet 11 inches
also, i would like to inform you that i have fatty liver (grade I). I'm not taking any medicines for fatty liver. I am doing brisk walking 2 kms in the morning and 1 kms in evening along with diet (low sugar, low salt, more fibre, vegetables, fruits + wheat products (instead of rice) for lunch)
also, i would like to make a mention that in the course of medication, i followed fruit / fibre rich diet due to which I reduced weight by about 7 to 8 kgs.
I have uploaded scanned reports (ECG, Echo cardiogram)
along with this, doctor has prescribed vorvas and maxepa (omega fatty acids) (dosage : 1 / day) capsules. I have attached the blood test report - lipid profile along with ecg and echo for your kind reference.

I am Dr. Mody and I would be addressing your concern .
I went through the attached reports , As you comprehend, Your Ecg and Echocardiography reports are with en normal .
Your lipids are just boderline high , the bad cholesterol ldl slightly high and the Good cholesterol Hdl slightly low .
Diet , exercise , losing weight and omega 3 fatty acid capsules should be fine . No medication needed for the same .
The grade 1 fatty lover is explained by you being overweight , concentrate in loosing weight at least till you reach less than 90 kg .

along at 2.5 mg is a far lesser dose as compared to previous combination , maybe your blood pressure was well controlled so he decided to decrease the dose .
Clapton per say is not just prescribed for hypertension, also its short acting . The most common reason it must have been given to you if you had some palpitation , because of extra beats and then they would have been more common at night .
No night dose is not harmful if given for the above mentioned reason .
My 1st click here would be a small dose of beta blocker .
so in toto your medication is grossly right

Doctor thanks for your expert opinion and advice. I would also like to mention that now my heart beat has come to normal. It had been more than 3 weeks now since doctor (general practitioner) prescribed calaptin 40 mg. Now, I am feeling normal and I'm able to do brisk walking for 30 minutes without any problem. My question is, can I taperoff calaptin 40 mg as I don't have any palpitation now . I came to know that calaptin should not be stopped abruptly and that it has to be gradually tappered off. If I should taper off calaptin 40 mg, Kindly indicate how it should be done. Also, should any hypertension drug needs to be substituted instead of calaptin 40 mg (which is being taken once a day at night time). Also, kindly give your expert advice on the jerky IVS motion mentioned in the echo cardiogram. (report attached)
Also, kindly let me know about the beta blocker medicine which you suggest.

I would enumerate my observations to answer your queries .
1) As I have mentioned that your fatty liver is largely because of your weight being on higher side , as you lose weight it would reverse over period of time .
Calaptin can be taken even when you have fatty liver grade 1 .
However I am not in favour of calaptin .
The main reason is calaptin 40 mg is a short acting drug , so there is a high chance that it would cause fluctuations in blood pressure over 24 hours , leading to erratic control and such fluctuation's are not Good , as they kill the whole purpose of effective blood pressure control . If you wish to continue calaptin consider taking an SR preparation .
Secondly Calaptin is not a very Good anti hypertensive and it is not recommend as first line drug .
As I thought calaptin must have been started because of palitations you had.
If they are well.controled you may slowly taper down calaptin to 20 mg for 1 week and than omit to be on safest side ( though this may not be necessary ).
An Alternative Beta blocker would be sustained release metoprolol ,
Prolomet Xl 12.5 mg once a day ( other alternative brands at similar doses ) .
Lastly about XXXXXXX IVS ,
I am not very convinced about the echo finding , Itso a subjective phenomenon and will not cause any problems ever .
In your ecg there is an incomplete Rbbb ( though not clear ) which may cause some jerkiness. However in both circumstances as I have mentioned it will not cause any problem ever.
