To answer your questions

Q I see food particles in my urine.

The only way to explain is there is perhaps an abnormal communication between your intestine and urinary system - suggest to see an urologist for the same. On the other hand, what you see may not be food particles but some sediments and casts. Routine urine examination will rule these out.

Q I am not able to sleep. I tend to get up after 4/5 hrs of sleep and hence am very irritated and angry all the time

There are multiple reasons for not getting the sleep

Subjective and objective causes

One of the common subjective cause is the stress and the solution for the same is to "de stress" by doing regular walking/exercise (45 minutes) and meditation
(45 minutes)
One of the common objective cause is illness like fever, exertion, pain etc and treating the same will solve the problem

Q I have lot of pigmentation on my skin on face , hands and back

Pigmentation again may be due to several causes and in diabetic patient one of the common cause is the insulin resistance and if you keep the sugar levels under control the same can be get rid off, and if the pigmentation is very severe than suggest to see a dermatologist

Q I was diagnosed as insulin resistance 3 years back and now diabetic 6 months ago.

Please do regular checkups, like sugar levels (FBS, PPBS) every month, and HBA1c levels every 3 months and Annual healthcheck up with the above tests and kidney, liver, eye check up and nerve study

Wish you good health
