What Do My Lab Test Results Indicate?

Question: I got fbs of 270 and rbs of 279 and a fatty lever symptom. No one in my family donot have diabetic conditions. This is the first time i have been tested with this result. Earlier (about 1.5 year ago) fbs was less than 100. I used to take green tea with honey and lemon on a daily basis. Food intake is not high. I am 49 years old.
Can u advise me.

Noted your concern.

Diagnosis of Diabetes is confirmed.
(Considering you are healthy & not on any medications)

In such a case I may suggest -

- Detailed physical examination & history taking that will help in deciding type of Diabetes (though Type 2 Diabetes is most likely consider presence of fatty liver & your age)

- Diabetic Diet : You should consult near by dietitian who can plan diet for you considering your Body Mass Index (BMI), food habits.

- Exercise : Usually 150 min per week of moderate degree physical exercise is advisable, though your effort intolerance need to be considered.

- Medications :
Considering Type 2 Diabetes I may suggest considering normal kidney & heart function -
Metformin 1500 to 2000mg per day in divided dose.
DPP4 inhibitors like Sitagliptin, Teneligliptin or Vildagliptin.
Small dose of long acting insulin at bed time can be preferred to begin with, though many patients reluctant for taking insulin.
To repeat fasting & post-prandial after 8 to 10 days and accordingly further changes in medication can be considered.

- Investigations
Complete Blood Count
HbA1c
Lipid Profile.
Renal function Test.
Liver Function Test.
Urine examination including urine ketone.
