My husband ( I have POA) was dx as vascular dem with Lewy bodies recently per MRI. He had no sx of dem but was having hallucinations of 1 yr duration. He was on Tramadol and pramipexole, which we recently discovered can cause hallucinations. He was started on Rivastigmine at the same time we d/c the 2 meds. His hallu. stopped and he has no sx of dem at all. Could he be misdiagnosed? I want to obtain copy of MRI and take it to a friend of mine for over read. What do you think?

Of course, if the symptoms have improved completely, then misdiagnosis may be a possibility. MRI can't confirm the diagnosis of LBD. It can be just supportive and more important is a historical and examination based diagnosis of Dementia. SPECT, in fact is a better tool for diagnosis. Besides, if the symptoms have improved vastly, then I would say no imaging is superior to the clinical presentation, especially in case of Dementia.
